REGULAR MEETING APRIL 12. 2016 132 <br />AWARD BID — RUM. VILLAGE NATURE CENTER ROOF REPLACEMENT PARK <br />BOND) <br />Mr. John Martinez, Parks Department, advised the Board that on March 8, 2016, bids were <br />received and opened for the above referenced project. After reviewing those bids, Mr. Martinez <br />recommended that the Board award the contract to the lowest responsive and responsible bidder <br />R. Yoder Construction, Inc., 27453 CR 150, P. 0. Box 69, Nappanee, Indiana 46550, in the <br />amount of $54,232.00 for the base bid only. Therefore, Ms. Maradik made a motion that the <br />recommendation be accepted and the bid be awarded as outlined above. Ms. Dorau seconded the <br />motion, which carried. <br />APPROVE CHANGE ORDER NO. 6 — WESTERN AVENUE STREFTSCAPES — PROJECT <br />NO. 114-085A (TIF INFRASTRUCTUREI <br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 6 on behalf <br />of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, Indiana 46614, <br />indicating the contract amount be increased by $3,717.57 for a new Contract sum, including this <br />Change Order, in the amount of $1,845,234.36. Upon a motion made by Ms. Maradik, seconded <br />by Mr. Relos and carried, the Change Order was approved. <br />APPROVE CHANGE ORDER NO. 3 — IGNITION PARK INFRASTRUCTURE PHASE 1C <br />DIVISION B — PROJECT NO 114-063A (RWDA TIF) <br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 3 on behalf <br />of Majority Builders, Inc., 62900 US 31 South, South Bend, Indiana 46614, indicating the <br />contract amount be increased by $27,658.87 for a new Contract sum, including this Change <br />Order, in the amount of $978,632.87. Upon a motion made by Ms. Maradik, seconded by Mr. <br />Relos and carried, the Change Order was approved. <br />APPROVE CHANGE ORDER NO. 5 — WESTERN AVENUE CORRIDOR IMPROVEMENTS <br />-PROJECT NO 114-033B (TIF BOND) <br />Mr. Gilot advised that Mr, Toy Villa, Engineering, has submitted Change Ordcr No. 5 on behalf <br />of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, Indiana 46614, <br />indicating the contract amount be increased by $2,222.00 for a new Contract sum, including this <br />Change Order, in the amount of $969,054.31, Upon a motion made by Ms. Maradik, seconded <br />by Mr. Relos and carried, the Change Order was approved. <br />APPROVE CHANGE ORDER NO. 1 1TINAL) AND PROJECT COMPLETION AFFIDAVIT <br />— STATE ROAD 23 SIDEWALK FROM PRAIRIE APARTMENTS TO LOCI JST ROAD PROJECT NO, 115-021 (MVH) <br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on <br />behalf of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, Indiana <br />indicating the contract amount be increased by $5,872.28 for a new contract sum, including this <br />Change Order, of $130,696.28. Additionally submitted was the Project Completion Affidavit <br />indicating this new final cost of $130,696.28. Upon a motion made by Mr. Relos, seconded by <br />Ms. Maradik and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were <br />approved. <br />TABLE REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — PRE - <br />QUALIFICATION FOR BODY SHOP REPAIR SERVICES — (VARIOUS DEPARTMENT'S <br />OPERATIONS BUDGETS) <br />In a memorandum to the Board, Mr. Jeff Hudak, Central Services, requested permission to <br />advertise for the receipt of bids for the above referenced services. Ms. Martin stated Mr. Hudak <br />has since asked that this request he tabled until further notice. Therefore, upon a motion made by <br />Mr. Relos, seconded by Ms. Maradik and carried, the above request was tabled. <br />APPROVAL OF REOUFST TO ADVERTISE FOR THE RECEIPT OF BIDS — TWO (2) <br />MORE OR LESS, 2016 OR NEWER FULL SIZE CARGO VANS (WASTE WATER AND <br />COMMUNITY INVESTMENT CAPITAL) <br />In a memorandum to the Board, Mr. Jeff Hudak, Central Services, requested permission to <br />advertise for the receipt of bids for the above referenced vehicles. Therefore, upon a motion <br />made by Mr. Relos, seconded by Ms. Maradik and carried, the above request was approved. <br />
