BID PROPOSAL REQUIREMENTS AND CONFORMITY <br />10.0 Record Maintenance <br />The company shall always maintain separate and accurate records in connection with the removal and <br />storage services it provides the City and shall make all records available to the City upon request. <br />Records to be maintained shall include the following: <br />a. Time, date, location of tow and officer authorizing the tow. <br />b. Make, model, vehicle license registration, and VIN number of towed vehicles. <br />c. Itemized billing of basic tow charges, special charges, storage charges, and any special <br />vehicle repair/care charges, i.e. window wrap, key charge, etc. <br />d. Signed release form by person claiming the vehicle. <br />e. If applicable, a complaint form, signed by the vehicle owner reporting any complaints <br />regarding charges, negligent vehicle damage, lost or stolen items from the vehicle or any <br />other related problems. <br />• Customer complaint forms will be posted in the office in a clearly marked space. <br />• All customer complaints will be mailed to the South Bend Police Department within five <br />(5) days of the filed complaint. <br />Asap Towing reserves the right to act under City Municipal Code, Chapter 20, Sec. 20-13 <br />(C) and Sec. 20-15 (C) The City, its agents, servants or employees, shall not be <br />responsible or liable in any manner for the damage incurred in towing, storage, <br />possession or disposition of such vehicle. <br />11.0 Risk <br />ASAP Towing holds indemnity and liability insurance under the U.S. Truck & Trailer Service, Inc., the <br />parent company, of ASAP Towing. Liability and property damage insurance covers up to $1,000,000.00 <br />and is held by Trumbull Insurance Company— certificate #877331622, policy #37UENOL2209. <br />ASAP assumes all risk in the event of damage, theft, fire, or otherwise, of the vehicles or any other <br />property towed, impounded, or stored by the company, its employees, or agents. <br />The company shall look solely and exclusively to the owner of the vehicle towed, impounded, or stored <br />for payment of services rendered and will hold no claims against the City of South Bend for costs or <br />nonpayment. <br />m <br />
