HRP <br />CONSTRUCTION <br />EXCAVATING-SEWERS-INATERMAINS <br />1 <br />1(14/07f;!629 go Lc LJC c�t2LTLCt II <br />June 25, 2019 <br />Josh Ehmer <br />City of South Bend — Engineering <br />County -City Building <br />227 West Jefferson Blvd. — Rm. 1316 <br />South Bend, IN 46601 <br />Re: Miami Basin — Change Order #2 - 2 Inch Conduit Lowering, 6 Inch Drainage Pipe <br />Extension <br />Dear Mr. Ehmer: <br />On April 17th, while removing the existing sidewalk the crew discovered a located 2 inch <br />conduit containing what we assume to be traffic and/or street lighting wires. The <br />conduit was in conflict with the proposed grades for the new access drives as well as <br />the sidewalk grading. Roger Nawrot directed the crew to lower the conduit where we <br />needed to, to obtain proper cover for grading of the walks and drives. We did so with a <br />crew of one Field Supervisor, laborer and operator using a Hitachi 300 excavator. With <br />that change to the scope we have incurred costs that resulted in an increase of the <br />following to the overall contract price: <br />$1,035.00 <br />On March 29th, during the earthwork portion of the project the crew discovered 2 each — <br />6 inch drainage pipes from the property immediately south of the basin that served as <br />what we determined are drywell overflow pipes. The crew extended the pipes into the <br />basin to continue to allow for the drywells to drain as necessary. The crew that was <br />onsite was a Field Supervisor, one laborer and two operators using excavator and dozer <br />for installation of the pipes.
