program as without this said evidence, then we will not be able to award the <br />contract to them. <br />On December 17, 1 spoke with Sara Grogan and Dwight Silva (Inland <br />Business Representative) requesting this information again and to express <br />the criticality. <br />They indicated they would be getting this information submitted. <br />On December 17th, I received an email from Jeremy Recklein (Inland <br />General Manager) with their in-house training guidelines. On December 18th <br />I replied asking if there is evidence of "participation in apprenticeship and <br />training programs". <br />Jeremy responded there is currently not an applicable labor category that is <br />consistent with the type of scope of work we conduct in the field, he also <br />indicated that they are members of various trade organizations all of which <br />do not have an apprentice ship programs applicable to their work. <br />2. SAK Construction — missing information (see attachment B) <br />a. Contractor's Bid for Public Work, Part II <br />i. Section 1. 3. Attach information regarding any failure to complete work <br />awarded to you and the location thereof. <br />ii. Section 11.2. Attach a listing of the names and addresses that you have used <br />on public works projects during past 5 years along with brief description of <br />the work done by each subcontractor. <br />iii. Section 11.3. If you intend to sublet any portion of work, attach the name and <br />address of each sub, equipment to be used and whether you will require a <br />bond. <br />The missing information that was requested on December 14, 2018. <br />The information was provided on December 1 Ph. <br />b. Responsible Bidder— Checklist <br />i. Bidder was considered pre -qualified during an un-project related submittal on <br />10/26/2018. <br />However, upon review of the Insituform and Inland Water Pollution Control, <br />Engineering looked closer at the initial submittal on 10/26/2018, and noticed <br />the provided "Evidence of participation in apprenticeship and training <br />programs" was noted to be from the state of Ohio. Furthermore, the <br />memorandum states that the document does not certify any relationship the <br />company has with the mentioned registered sponsors. <br />3. Insituform — missing information <br />a. Responsible Bidder —Checklist <br />i. Part 11. a. (iii) Prequalification checklist (for bidders that are not pre -qualified) <br />By checking this box, I hereby acknowledge that copies of applicable <br />certificates or standards for training programs applicable to the work <br />performed don the project may be requested at any time and shall be <br />furnished upon request. <br />The bid package from Insituform acknowledged this statement. <br />ii. Part 11. b. (vi) — Evidence of participation in apprenticeship and training <br />programs, applicable to the work to be performed on the project, which are <br />approved by and registered with the United States Department of Labor's <br />Office of Apprenticeship, or its successor organization. <br />In their bid, they stated that. Insituform is full staffed and capable of meeting <br />all of the project requirements to complete this job for the City of South Bend. <br />Bidder pulls trained labor from Locals 120 and 103. Bidder does not have <br />apprenticeships due to the specific skill set for our services. <br />
