Martin, Clerk <br />FROM: Sue Ellen Doudrick, P.E., Assistant City Engineer <br />SUBJECT: Project No. 118-093 Sewer & Manhole Rehabilitation <br />DATE: December 20, 2018 <br />On December 11 2018, the Board of Public Works opened and read bids for the above project. <br />The Division of Engineering reviewed the documents and found each bidder failed to submit <br />information required per the standard documents. I worked with each of the low bidders to obtain the <br />required information. Separate from the award memorandum to the Board of Public Works dated <br />December 18, 2018, 1 wanted to document the following information with the Board of Public Works <br />to compile all data. <br />Inland Waters Pollution Control — missing information (See attachment A): <br />a. Contractor's Bid For Public Work, Part II <br />i. Section II — If you intend to sublet any portion of the work, attach the name <br />and address of each subcontractor. A listing must be provided prior to <br />contract approval. <br />The original bid stated that they are unable to provide a listing. <br />I reached out to Inland on 12/14/2018 and requested this. <br />On 12117, Inland Water provided a listing. <br />b. Responsible Bidder— Checklist <br />i. Part 11. a. (iii) Prequalification checklist (for bidders that are not pre -qualified) <br />By checking this box, I hereby acknowledge that copies of applicable <br />certificates or standards for training programs applicable to the work <br />performed don the project may be requested at any time and shall be <br />furnished upon request. <br />The bid package from Inland Waters acknowledged this statement. <br />ii. Part II. b. (vi) — Evidence of participation in apprenticeship and training <br />programs, applicable to the work to be performed on the project, which are <br />approved by and registered with the United States Department of Labor's <br />Office of Apprenticeship, or its successor organization. <br />In their bid, they stated that: Inland Water Pollution control, Inc. does not <br />currently employ apprenticeship programs or training programs registered <br />with the United States Department of Labor's Office of Apprenticeship. <br />On December 14,1 reached out to Sara Grogan (Inland Associate Estimator) <br />informing her that it is critical for them to look closer into their training <br />
