Field repairs of solid state equipment will be limited to <br />replacement of faulty equipment and returning such equipment to the <br />INDOT in exchange for replacement equipment. <br />g. Trucks: <br />1. Pick-up truck used by unskilled labor or as parts truck. <br />2. Aerial truck- vehicle used and charged for as "Base Vehicle" tech <br />(skilled labor) dispatches; equipment that is capable of lifting one <br />or more workers and material for the purpose of overhead work. <br />3. Boom/Auger Truck: Truck mounted equipment capable of lifting and <br />setting poles or mast arms, and drilling holes. <br />h. Unskilled Labor: Common Laborer, Escort Service, Flag Person <br />7. INVENTORY REPORTING <br />a. INDOT will provide to the CITY a "Stock Transfer Report" listing <br />materials and equipment provided to the CITY. A list of the items that <br />the department shall furnish to the CITY is listed in Exhibit "L" INDOT <br />Provided Materials. A received slip shall be verified and signed by a <br />delegated representative of the CITY and given to INDOT. INDOT will <br />provide to the CITY a "Stock Transfer Report" listing materials and <br />equipment returned by the CITY. A returned slip will be verified and <br />signed by a representative of INDOT and given to the CITY. <br />b. The CITY shall supply an itemized report of all material supplied by <br />INDOT that is in the following status: <br />1. Stock on hand at beginning of period. <br />2. Stock obtained during the period. <br />3. Stock used during the period. <br />4. Stock on hand at the end of the period. <br />5. Stock returned to Indiana Department of Transportation. <br />c. Such report shall reflect the date and where INDOT materials were used, <br />received, or transferred. Reports shall be submitted within 14 days <br />following the let day of the months April, July, October, and January <br />for all material reflecting activity for that three month period. At <br />the end of the Contract, reports for all material shall be submitted. <br />d. In addition, the CITY will be required to supply the locations where <br />the materials are used, by type of material. The format of this report <br />shall be determined prior to the effective starting date of this <br />Page 7 of 53 Pages <br />
