REGULAR MEETING

<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION
<br />AFFIDAVIT - HOWARD PARK SIDEWALK REPLACEMENT PROJECT
<br />Mrs. DeClercq advised that McIntyre & Jones Construction Company,
<br />Inc., 2526 West 6th Street, Mishawaka, Indiana, has submitted
<br />Change Order No. 1 (Final) indicating that the Contract amount,
<br />for the above referred to project, be increased by $1,047.64 for
<br />a new Contract sum including this Change Order in the amount of
<br />$10,351.10. Additionally submitted was the Project Completion
<br />Affidavit indicating this new final cost. Upon a motion made by
<br />Mrs. DeClercq, seconded by Ms. Humphreys and carried, Change
<br />Order No. 1 (Final) and the Project Completion Affidavit were
<br />approved and the appropriate three-year Maintenance Bond as
<br />submitted was accepted for filing.
<br />APPROVE CHANGE ORDER NO. 1 AND PROJECT COMPLETION AFFIDAVIT -
<br />IRONWOOD-IRELAND RETENTION BASIN IMPROVEMENTS PROJECT
<br />Mrs. DeClercq stated that Small, Inc., 25190 State Road 4, North
<br />Liberty, Indiana, has submitted Change Order No. 1 for the above
<br />referred to project indicating that the Contract amount be
<br />increased by $17,939.00 for a new Contract sum including this
<br />Change Order in the amount of $343,660.50. Additionally
<br />submitted was the Project Completion Affidavit indicating this
<br />new final cost. Upon a motion made by Mrs. DeClercq, seconded by
<br />Ms. Humphreys and carried, Change Order No. 1 and the Project
<br />Completion Affidavit were approved and the appropriate three-year
<br />Maintenance Bond as submitted was accepted for filing.
<br />PROJECT COMPLETION AFFIDAVIT - PAINTING AND FINISHING AT
<br />THE WASTEWATER TREATMENT PLANT PROJECT (3113 RIVERSIDE DRIVE)
<br />Mrs. DeClercq indicated that Interstate Decorating Service, 140
<br />North Meade Street, P.O. Box 3684, South Bend, Indiana, has
<br />submitted to the Board a Project Completion Affidavit for the
<br />above referred to project indicating a final cost of $88,385.75.
<br />It is noted that Interstate Decorating Services shall guarantee
<br />all work to be free from defects in workmanship and materials for
<br />a period of one (1) year from June 28, 1991 the date of
<br />substantial completion. Therefore, Mrs. DeClercq made a motion
<br />that. the Project Completion Affidavit be accepted and approved.
<br />Ms. Humphreys seconded the motion which carried.
<br />AWARD BID - TWENTY-FIVE (25), MORE OR LESS, 1992 POLICE PATROL
<br />AUTOMOBILES
<br />Mrs. DeClercq noted that the Board is in receipt of a letter from
<br />Mr. Phil St. Clair, Director, Division of Equipment Services,
<br />advising that he has reviewed the bids received on December 16,
<br />1991 for the above referred to vehicles. Mr. St. Clair noted
<br />that. Gates Chevrolet Corporation, 401 South Lafayette Boulevard,
<br />South Bend, Indiana, was the low bidder at $12,234.22 per
<br />vehicle. He also noted that the bids of Gates Chevrolet and
<br />Jordan Ford met or exceeded specifications but that the bid of W.
<br />Hare did not meet engine specifications. Therefore, Ms.
<br />Humphreys made a motion that the bid of Gates Chevrolet
<br />Corporation in the amount of $12,234.22 per vehicle be awarded
<br />for the purchase of twenty-five (25) 1992 Chevrolet Caprice 4
<br />door sedans. Mrs. DeClercq seconded the motion which carried.
<br />AWARD QUOTATION - CENTRAL BUSINESS DISTRICT MAIN AND WESTERN
<br />LANDSCAPING
<br />In a Memorandum to the Board, Mr. Carl P. Littrell, Director,
<br />Division of Engineering, advised that on December 16, 1991, the
<br />Board received and opened Quotations from three (3) landscaping
<br />contractors for the above referred to project. Mr. Littrell
<br />stated that each of the contractors rounded off the extensions in
<br />their bids and an attachment to the Quotation of Landscapes
<br />Unlimited's Quotation form corrected their price for the trees.
<br />That unit price is shown in the tabulation. These corrections
