<br />made a motion that the recommendation be accepted and the quotation be awarded as outlined

<br />AWARD QUOTATION — TRAFFIC CAMERAS AT PORTAGE AVENUE FOREST

<br />SERVICESlTRAFFIC SIGNAL MAINT )
<br />Ms. Blair Bennett, Engineering, advised the Board that on May 19, 2016, quotations were
<br />received and opened for the above referenced project. After reviewing those quotations, Ms. —
<br />Bennett recommended that the Board award the contract to the lowest responsive and responsible
<br />bidder, Michiana Contracting, Inc., 7843 Lilac Road, Plymouth, Indiana 46563, in unit prices,
<br />not to exceed $31,000. Therefore, Ms. Maradik made a motion that the recommendation be
<br />accepted and the quotation be awarded as outlined above. Mr. Relos seconded the motion, which
<br />carried
<br />APPROVE CHANGE ORDER NO. 6, PRIMARY CLARIFIER REHABILITATION AND
<br />EQUIPMENT UPGRADES — PROJECT NO 111-071 (UTILITIESlWASTEWATER
<br />CAPITAL)
<br />Mr. Gilot advised that Mr. Jacob Klosinski, Environmental Services, has submitted Change
<br />Order No. 6 on behalf of L. D. Docsa Associates, Inc.., 1605 King Highway, Kalamazoo,
<br />Michigan 49001, indicating the Contract amount be increased by $59,686.73 for a new contract
<br />sum, including this Change Order, in the amount of $5,150,540.51 and the schedule be increased
<br />by 203 days. Upon a motion made by Mr. Relos, seconded by Ms. Maradik and carried, the
<br />Change Order was approved.
<br />APPROVE CHANGE ORDER NO 1 (FINAL) AND PROJECT COMPTETION AFFIDAVIT
<br />—DEMOLITION OF PNA 83 —PROJECT NO 115-118 (RWDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of Green Demolition, Inc., 1427 W. Dickens Ave., Ste. 200, South Bend, Indiana
<br />indicating the contract amount be decreased by $10,000 for a new contract sum, including this _
<br />Change Order, of $72,800. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost of $72,800. Upon a motion made by Ms. Maradik, seconded by
<br />Mr. Relos and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were
<br />approved.

<br />Mr, Gilot advised that Mr, Jacob Klosinski, Environmental Services, has submitted Change
<br />Order No. 1 (Final) on behalf of Stewart Spreading, Inc„ 3870 N. Illinois Rte. 71, Sheridan,
<br />Illinois 60551, indicating the contract amount be increased by $97,570 for a new contract sum,
<br />including this Change Order, of $599,010. Additionally submitted was the Project Completion
<br />Affidavit indicating this new final cost of $599,010. Upon a motion made by Ms. Maradik,
<br />seconded by Mr. Relos and carried, Change Order No. 1 (Final) and the Project Completion
<br />Affidavit were approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — 2016
<br />JANITORIAL CLEANING SUPPLIES SPEC Z (VARIOUS DEPARTMENT'S
<br />OPERATIONS BUDGETS)
<br />In a memorandum to the Board, Ms. Toni Laisure, Central Services, requested permission to
<br />advertise for the receipt of bids for the above referenced supplies. Therefore, upon a motion
<br />made by Ms. Maradik, seconded by Ms. Dorau and carried, the above request was approved. —
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS AND TITLE
<br />SHEET — MARRIOTT HOTEL SITE DEVELOPMENT AT THE HALL OF FAME PHASE I
<br />PROJECT NO 116-021 (RWDATIF)
<br />In a memorandum to the Board, Mr. Richard Estes, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Also presented at this time for
<br />approval and execution was the Title Sheet. Upon a motion made by Mr. Relos, seconded by Ms.
<br />Maradik and carried, the above request to advertise was approved, and the Title Sheet was
<br />approved and signed.
