<br />APPROVE CHANGE ORDER NO. I (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— 2015 CURB & SIDEWALK PROGRAM, DIVISION C —PROJECT NO. 115-069 (MVH)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of Walsh & Kelly, Inc., 24358 State Road 23, South Bend, Indiana 46614 indicating the
<br />contract amount be decreased by $11,497.00 for a new contract sum, including this Change
<br />Order, of $123,278.00. Additionally submitted was the Project Completion Affidavit indicating
<br />this new final cost of $123,278.00. Upon a motion made by Mr. Relos, seconded by Ms. Doran
<br />and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved.

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf Ortiz Concrete indicating the contract amount be decreased by $5,026.50 for a new
<br />contract sum, including this Change Order, of $67,113.50. Additionally submitted was the
<br />Project Completion Affidavit indicating this new final cost of $67,113.50. Upon a motion made
<br />by Mr. Relos, seconded by Ms. Dorau and carried, Change Order No. 1 (Final) and the Project
<br />Completion Affidavit were approved.
<br />APPROVE CHANCE ORDER NO 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />—DEMOLITION OF 1124 W. GRACE —PROJECT NO 115-119 (RWDATIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. I (Final) on
<br />behalf of Torok Excavating, 825 Fellows, South Bend, Indiana 46637 indicating the contract
<br />amount be decreased by $5,000.00 for a new contract sum, including this Change Order, of
<br />$10,100.00. Additionally submitted was the Project Completion Affidavit indicating this new
<br />final cost of $10, 100.00. Upon a. motion made by Mr. Relos, seconded by Ms. Doran and carried,
<br />Change Order No. 1 (Final) and the Project Completion Affidavit were approved.
<br />APPROVE CHANGE ORDER NO 1 (FINAL) AND PROJECT COMPLE NON AFFIDAVIT

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of C&E Excavating, Inc., 53767 CR 9N, Elkhart, Indiana 46514, indicating the contract
<br />amount be decreased by $10,430.25 for a new contract sum, including this Change Order, of
<br />S165,707.25. Additionally submitted was the Project Completion Affidavit indicating this new
<br />final cost of S165,707.25. Upon a motion made by Mr. Relos, seconded by Ms. Doran and
<br />carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — 2014/2015 YARD LAMPPOST
<br />PROGRAM - PROJECT NO. 115-077 (LOSS RECOVERY)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Martell Electric LLC, 4601 Cleveland Road, South Bend, Indiana 46628,
<br />for the above referenced project, indicating a final cost of $79,650.00. Upon a motion made by
<br />Mr. Relos, seconded by Ms. Doran and carried, the Project Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — CENTURY CENTER SOUTH ENTRY
<br />IMPROVEMENTS -PROJECT NO. 115-014 (HOTELJMOTEL TAX)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Ziolkowski Construction, Inc., 4050 Ralph Jones Drive, South Bend,
<br />Indiana 46628, for the above referenced project, indicating a final cost of $78,162.00, Upon a
<br />motion made by Mr. Relos, seconded by Ms. Doran and carried, the Project Completion
<br />Affidavit was approved. _

<br />Mr. Gilot advised that Mr. Jamb Klosinski, Environmental Services, has requested the Board
<br />table this Project Completion Affidavit until further notice. Therefore, upon a motion made by
<br />Mr. Gilot, seconded by Mr. Relos and carried, the Project Completion Affidavit was tabled.
