<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of Indiana Earth, Inc., 10343 McKinley Highway, Mishawaka, Indiana, 46561, indicating the
<br />Contract amount be decreased by $9,300.00 for a new contract total of $187,350.00. Upon a
<br />motion made by Mr. Relos, seconded by Ms. Dorau and carried, the Change Order was
<br />approved.

<br />Contract amount be increased by $3,168.00 for a new contract total of $190,518.00. Upon a
<br />motion made by Mr. Relos, seconded by Ms. Doran and carried, the Change Order was
<br />approved.

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of Martell Electric, 4601 Cleveland Road, South Bend, Indiana, 46628, indicating the Contract
<br />amount he increased by $13,789.25, for a new contract total of $86,364.25 and a sixty (60) day
<br />extension. Upon a motion made by Mr. Relos, seconded by Ms. Doran and carried, the Change
<br />Order was approved.
<br />APPROVE CHANGE ORDER NO 1 - TWYCKENHAM TRAFFIC CALMING — PROJECT
<br />NO. 115-092 (NEIGHBORHOOD TRAFFIC CALMING)
<br />Mr. Gilot advised that Ms. Blair Bennett, Engineering, has submitted Change Order No. I on
<br />behalf of Martell Electric, LLC, 4601 Cleveland Road, South Bend, Indiana 46628, indicating
<br />the contract amount be increased by $1,094.00 for a modified contract sum, including this
<br />Change Order, in the amount of $15,786.00 and a forty-five (45) day extension. Upon a motion
<br />made by Mr. Roles, seconded by Ms. Doran and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of Walsh & Kelly, Inc., 24358 State Road 23, South Bend, Indiana 46614 indicating the
<br />contract amount be decreased by $7,024.00 for a new contract sum, including this Change Order,
<br />of $93,306.00. Additionally submitted was the Project Completion Affidavit indicating this new
<br />final cost of $93,306.00. Upon a motion made by Mr. Relos, seconded by Ms. Domu and carried,
<br />Change Order No. 1 (Final) and the Project Completion Affidavit were approved.
