<br />C&S Masonry Restoration in the amount of S 139,000.00 for the Base Bid Phase I, Phase B, and
<br />the Alternate Bid Phase I. Mr. Carey explained that the bids contained additional unit price work
<br />that was not awarded due to budget constraints. Ile asked that the Board rescind the award of
<br />Phase II in the amount of $41,000.00 and award 40,773 square feet of Items Nos. 2 and 3, and
<br />845 Linear Feet for Item No. 4 for $148,537.32 bringing the new total award to $246,537.32. Mr.
<br />Carey noted that C&S Masonry is still the lowest responsive bidder. Therefore, Mr. Relos made
<br />a motion that the recommendation be accepted and the bid be rescinded and awarded as outlined
<br />above. Ms. Maradik seconded the motion, which carried.
<br />AWARD QUOTATION — TWYCKENHAM TRAFFIC CALMING — PROJECT NO. 115-092
<br />(2015 NEIGHBORHOOD TRAFFIC CALMING)
<br />Ms. Blair Bennett, Engineering, advised the Board that on December 3, 2015, quotations were
<br />received and opened for the above referenced project. After reviewing those quotations, Ms.
<br />Bennett recommended that the Board award the contract to the lowest responsive and responsible
<br />bidder, Martell Electric, LLC, 4601 Cleveland Rd., South Bend, Indiana, in the amount of
<br />$14,692.00. Therefore, Ms. Maradik made a motion that the recommendation be accepted and
<br />the quotation be awarded as outlined above. Mr. Relos seconded the motion, which carried.
<br />AWARD QUOTATION — TRAFFIC SIGNAL IMPROVEMENTS — PROJECT NO. 115-131
<br />(TRAFFIC SIGNAL REPAIRS & MAINTENACE)
<br />Mr. Roger Nawrot, Engineering, advised the Boardthat on December 3, 2015, quotations were
<br />received and opened for the above referenced project. After reviewing those quotations, Mr.
<br />Nawrot recommended that the Board award the contract to the lowest responsive and responsible
<br />bidder, Pemberton Davis Electric, Inc., 916 E. McKinley Ave., Mishawaka, Indiana, in unit
<br />prices not to exceed $47,176.00. Therefore, Mr. Relos made a motion that the recommendation
<br />be accepted and the quotation be awarded as outlined above. Ms. Maradik seconded the motion,
<br />which carried.
<br />APPROVE CHANGE ORDER. NO. 2 — CENTURY CENTER SOUTH ENTRY
<br />IMPROVEMENTS —PROJECT NO. 115-014 (HOTEL/MOTEL TAX) _
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering. has submitted Change Order No. 2 on behalf
<br />of Ziolkowski Construction, Inc., 4050 Ralph Jones Drive, South Bend, Indiana 46628,
<br />indicating the Contract amount be increased by $799.00 for a new Contract sum, including this
<br />Change Order, in the amount of $78,162.00. Upon a motion made by Ms. Maradik, seconded by
<br />Mr. Relos and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO 2 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— HILL STREET IMPROVEMENTS PHASE II — PROJECT NO 113-024A (REDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 2 (Final) on
<br />behalf of Rieth-Riley Construction Co., Inc., 25200 SR 23, South Bend, Indiana 46614,
<br />indicating the contract amount be decreased by $2,289.00 for a new contract sum, including this
<br />Change Order, of $121,813.50. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost of $121,813.50. Upon a motion made by Ms. Maradik, seconded by
<br />Mr. Relos and carried, Change Order No. 2 (Final) and the Project Completion Affidavit were
<br />approved.
<br />APPROVE CHANGE ORDER NO 2 (FINAL) AND PROJECT COMPLETION AFFIDAVIT_

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 2 (Final)
<br />on behalf of Selge Construction Co., Inc., 2833 South 11' St., Niles, Michigan 49120, indicating
<br />the contract amount be increased by $4,039.91 for a new contract sum, including this Change
<br />Order, of $257,718.06. Additionally submitted was the Project Completion Affidavit indicating
<br />this new final cost of $257,718.06. Upon a motion made by Ms. Maradik, seconded by Mr. Relos
<br />and carried, Change Order No. 2 (Final) and the Project Completion Affidavit were approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — 2015 PARKING GARAGE DECK
<br />COATING (WAYNE ST) - PROJECT NO. 115-074B (PARKING GARAGE FIJND)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of RAM Construction Services, Inc., 13800 Eckles Road, Livonia, MI 48150,
