<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, Indiana 46614,
<br />indicating the Contract amount be increased by $20,848.71 for a new Contract sum, including
<br />this Change Order, in the amount of $572,432.46, Upon a motion made by Mr. Relos, seconded
<br />by Ms. Maradik and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 - BARTLETT ST. PHASE 1 & MEMORIAL NORTH
<br />PARKING LOT - PROJECT NO 114-047 (MEDICAL TIF AND RWDA TIE)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. I on behalf
<br />of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, Indiana 46614,
<br />indicating the Contract amount be increased by $8,338.98 for a new Contract sum, including this
<br />Change Order, in the amount of $1,915,306.65. Upon a motion made by Mr. Retos, seconded by
<br />Ms. Maradik and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 6 - TGNITION PARK INFRASTRUCTURE. PHASE 1C,
<br />DIVISION A - PROTECT NO 114-063A (RWDA TIF)
<br />Mr. Oiler advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 6 on behalf
<br />of Walsh & Kelly, Inc., 24358 State Road 23, South Bend, Indiana 46601, indicating the
<br />Contract amount be increased by $67,984.40 for a new Contract sum, including this Cbange
<br />Order, in the amount of $3,979,019.91. Upon a motion made by Mr. Relos, seconded by Ms.
<br />Maradik and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />- MADISON STREET & MARION STREET ONE WAY TO TWO WAY COWERSION -
<br />PROJECT NO 114-034 (TIF INFRASTRUCTURE BOND)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering has submitted Change Order No. 1 (Final) on
<br />behalf of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, Indiana 46614
<br />indicating the contract amount be decreased by $8,839.45 for a new contract sum, including this
<br />Change Order, of $568,330,60. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost of $568,350.60. Upon a motion made by Ms. Maradik, seconded by
<br />Mr. Relos and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were -'
<br />approved.
<br />APPROVE CHANGE ORDER NO 3 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />- SOUTHWOOD SEWER SEPARATTON - PROJECT NO. 112-042 (2012 SEWER BOND)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering has submitted Change Order No. 3 (Final) on
<br />behalf of Walsh & Kelly Construction, Inc., 24358 State Road 23. South Bend, Indiana 46614
<br />indicating the contract amount be increased by $6,251.46 for a new contract sum, including this
<br />Change Order, of $2,015,112.95. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost of S2,015,112.95. Upon a motion made by Ms. Maradik, seconded
<br />by Mr. Relos and carried, Change Order No. 3 (Final) and the Project Completion Affidavit were
<br />approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR TIIE RECEIPT OF BIDS AND TITLE
<br />SKEET - COLFAX AVENUE AND HILL STREET IMPROVEMENTS - PROJECT NO. 114-
<br />018 (REDA TIF)
<br />In a memorandum to the Board, Mr. Roger Nawrot, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Also presented at this time for
<br />approval and execution was the Title Sheet. Upon a motion made by Mr. Relos, seconded by Ms.
<br />Maradik and carried, the above request to advertise was approved, and the Title Sheet was
<br />approved and signed
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS - DEMOLITION
<br />OF VACANT & ABANDONED HOUSES, PHASE VIII - PROJECT NO. 115-128 (COIT)
<br />In a memorandum to the Board, Mr, Michael Carey, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Upon a motion made by Ms.
<br />Maradik, seconded by Mr. Relos and carried, the above request to advertise was approved.
<br />APPROVE TITLE SIIEET - CLEVELAND ROAD AND DIDAM BOULEVARD SEWER
<br />EXTENSION-PROJECTNO. 115-127
<br />Mr. Gilot advised that the Title Sheet for the above referenced project was being presented at this
<br />time for execution. He noted the City will not bid this project, General Sheet Metal will do the
