<br />AWARD QUOTATION — KENNEDY PARK BASKETBALL COURT INSTALLATION
<br />(PARK BOND)
<br />Mr. John Martinez, Park Department, advised the Board that on September 22, 2015, quotations
<br />were received and opened for the above referenced project. After reviewing those quotations,
<br />Mr. Martinez recommended that the Boardaward the contract to the lowest responsive and
<br />responsible bidder, Ancon Construction Co., Inc., 2146 Elkhart Road, Goshen, Indiana 46526, in
<br />the amount of $31900.00. Therefore, Mr. Relos made a motion that the recommendation be
<br />accepted and the quotation be awarded as outlined above. Ms. Maradik seconded the motion,
<br />which carried.
<br />APPROVE CHANGE ORDER NO. 1 — DEMOLITION OF VACANT & ABANDONED
<br />HOUSES DIVISION F — PROJECT NO. 115-047 LOSS RECOVERY FUND
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of Green Demolition Contractors, Inc., 1427 Dickens Street, Chicago, Illinois, 60614, indicating
<br />the Contract amount be increased by $5,005,00 for a new Contract sum, including this Change
<br />Order, in the amount of $375,405.00. Upon a motion made by Ms. Maradik, seconded by Mr.
<br />Relos and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — DEMOLITION OF VACANT & ABANDONED
<br />HOUSES DIVISIONA— PROJECT No. 115-047 LOSS RECOVERY FUND
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. I on behalf
<br />of Green Demolition Contractors, Inc., 1427 Dickens Street, Chicago, Illinois, 60614, indicating
<br />the Contract amount be increased by $3,135.00 and extended one (1) day for a new Contract
<br />sum, including this Change Order, in the amount of $263,735.00. Upon a motion made by Ms.
<br />Maradik, seconded by Mr. Relos and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — PARKING GARAGE DECK COATINGS — PROJECT
<br />NO 115-074B (PARKING GARAGE FUND)
<br />Mr. Gilot advised that Ms. Blair Bennett, Engineering, has submitted Change Order No. 1 on
<br />behalf of RAM Construction Services, Inc., 13800 Eckles Road, Livonia, Michigan 48150,
<br />indicating the Contract amount be increased by $33,480.00 for a new Contract sum, including
<br />this Change Order, in the amount of $500,284.44. Upon a motion made by Mr. Relos, seconded
<br />by Ms. Maradik and carried, the Change Order was approved.

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of Selge Construction Co., Inc., 2811 South 1 Is' Street, Niles, Michigan 49120, indicating
<br />the contract amount be decreased by $3,517.70 for a new contract sum, including this Change
<br />Order, of $118,817.40. Additionally submitted was the Project Completion Affidavit indicating
<br />this new final cost of $118,817.40. Upon a motion made by Mr. Relos, seconded by Ms.
<br />Maradik and carried, Change Order No. I (Final) and the Project Completion Affidavit were
<br />approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — REMOVAL OF LEAD BASED PAINT
<br />AT IVY TOWER PHASE II — PROJECT NO 115 030 (RWDA TIF)
<br />Mx. Gilot advised that Mr. toy Villa. Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Interstate Environmental Services, Inc., 337 West 806 N, Valparaiso,
<br />Indiana 46385, for the above referenced project, indicating a final cost of $302,950.00. Upon a
<br />motion made by Mr. Roles, seconded by Ms. Maradik and carried, the Project Completion
<br />Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — IGNITION PARK SITE
<br />IMPROVEMENTS — BUILDINGS 1 & 2 — PROJECT NO 114-050 (RWDA TIFI
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Majority Builders, Inc., 62900 US 31 South, South Bend, Indiana, 46614,
<br />for the above referenced project, indicating a final cost of $90,479.54. Upon a motion made by
<br />Mr. Relos, seconded by Ms. Maradik and carried, the Project Completion Affidavit was
<br />approved.
