<br />APPROVE PROJECT COMPLETION AFFIDAVIT — CATALYST ONE & TWO BUILDING
<br />FACADE ACM PANEL WALLS — PROJECT NO. 115-006 (RWDA TIE)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Shaffner, Heaney Associates, Inc., 2508 South Main Street, South Bend,
<br />Indiana 46614, for the above referenced project, indicating a final cost of $409,851,00. Upon a
<br />motion made by Ms. Maradik, seconded by Mr. Relos and carried, the Project Completion
<br />Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — WAYNE STREET PARKING —
<br />GARAGE FACADE REPLACEMENT — PROJECT NO 114-027R (PARKING GARAGE
<br />FUND)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Slatile Roofing and Restoration, Inc., 1703 South Ironwood, South Bend,
<br />Indiana 46613, for the above referenced project, indicating a final cost of $263,564.00. Upon a
<br />motion made by Mr. Relos, seconded by Ms. Maradik and carried, the Project Completion
<br />Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — LINCOLN WAY WEST CORRIDOR
<br />IMPROVEMENTS DIVISION B —PROJECT NO. 114-033A (LOSS RECOVERY FUND)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Martell Electric, LI.C, 4601 Cleveland Road, South Bend, Indiana 46628,
<br />for the above referenced project, indicating a final cost of $217,790.40. Upon a motion made by
<br />Ms. Maradik, seconded by Mr. Relos and carried, the Project Completion Affidavit was
<br />approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE. RECEIPT OF BIDS — KITCHEN
<br />EQUIPMENT FOR CENTURY CENTER — PROJECT NO. 115-018 (HOTEL/MOTF.i. TAX)
<br />In a memorandum to the Board, Mr. Michael Carey, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Therefore, upon a motion made
<br />by Mr. Relos, seconded by Ms. Maradik and carried, the above request was approved.
<br />APPROVAL OF REOUEST TO ADVERTISE FOR THE RECEIPT OF BIDS —DEMOLITION
<br />OF POLISII NATIONAL ALLIANCE NO. 83 — PROJECT NO. 115-118 (RWDA TIF)
<br />In a memorandum to the Board, Mr. Michael Carey, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Therefore, upon a motion made
<br />by Ms. Maradik, seconded by Mr. Relos and carried, the above request was approved.
<br />APPROVE INDOT TITLE SHEET— OLIVE STREET AND SAMPLE STREET
<br />Mr. Gilot advised that the Title Sheet for the above referenced INDOT project was being
<br />presented at this time for the City's signature. Upon a motion made by Ms. Maradik, seconded
<br />by Mr. Relos and carried, the above referenced Title Sheet was approved and signed.
<br />ADOPT RESOLUTION NO. 57.2015 — RESOLUTION OF THE SOUTH BEND BOARD OF
<br />PUBLIC WORKS ON DISPOSAL OF UNFIT AND/OR OBSOLETE PROPERTY
<br />Upon a motion made by Mr. Relos, seconded by Ms. Maradik and carried, the following
<br />Resolution was adopted by the Board of Public Works:

<br />RESOLUTION OF THE SOUTH BEND BOARD OF PUBLIC WORKS
<br />ON DISPOSAL OF UNFIT AND/OR OBSOLETE PROPERTY
<br />WHEREAS, it has been determined by the Board of Public Works that the following
<br />property is unfit for the purpose for which it was intended and is no longer needed by the City of
<br />South Bend:

<br />WHEREAS, Indiana Code 5-22-22-1 permits and establishes procedures for disposal of
<br />personal property which is unfit for the purpose for which it was intended and which is no longer
