1. _Name of Applicant 2. Number of Indiana-Certificate of license for the year in which the person applies for a. City License. S. Location of Shop or place where_ 'it is` proposed to carry on business. INSPECTION OF PROSPECTIVE -PLACE OF BUSINESS. It shall -be the duty of the Health Director and the Inspector of Health to investigate such proposed place of business, and if-it-shall appear to the Health Director, and the Inspector of Health, that the applicant has complied with all the provisions of this Ordinance and the Indiana State Barber Law, and any act con- cerning cosmetology, and that the place where the applicant pro- poses to carry on business is a proper and sanitary place there- for within the provisions of this Ordinance, the Health Director and the Inspector of Health, shall endorse a° recommendation to the Qity..Comptroller that .a. license be issued to such,app- licant and upon the receipt °of such recommendation, it shall ' be the duty of the City Comptroller to issue such license: Applications must be submitted within 15 days after the eff- ective date of this Ordinance. This procedure for granting license shall apply only -to` such persons as are found to be'practieing the profession of Barbering in the City of South Bend at the time this'- 'Ordinance becomes effective. Any person entering the City of South Bend after this ordinance becomes effective, who may be desirous of procuring.a license - hereunderi shall appear in person at the office of the Health Director in the City of South Bend. The Health Director-shall ascertain the name of the applicant, - -' number of state certificate of license, and location of Shop or place where it is proposed to carry on business. If the appli- cant is s.Journeman he.shall give the name of the Shop owner who desires the service of the Applicant Applicant shall pay $2.00 in advance on applying for license. If the applicant shall have qualified, then on recommendation of the Health Director, the City Comptroller shall issue'a City License to` the Applicant. SECTION FOUR. LICENSE FEE AND RENEWAL. Each and every, person now engaged in the business of Barbering it the City of South Bend or-operating a Barber Shop,' shall pay to' the City of South Bend, through the City Comptroller, a license fee of. $2.00 in advance, said license to terminate one year from date of issuance,`and vay be reneged upon expiration, by payment in advance of an annual fee of $2.00. Any Barber who is now a resident of the City of South Bend, shall apply for license within fifteen days from the effec- tive date. of this ordinance, and shall pay a license fee of $2.00 in advance on applying for a license. SECTION FIVE. LICENSE NOT T.0 BE TRANSFERRED. Such license shall not be transferable, but nothing herein contained shall present any person licensed under the terms of this Ordinance from using his or her license in case or removal of his or her business to another location within the City Limits of the City of South Bend. = Any license issued under' the terms of this Ordinance shall" "be placed in a .conspicuous place in said Barber Shop.
