REGULAR MEETING APRIL 14.2015 110 TABLE CHANGE ORDER NO. 2 (FINAL) AND PROJECT COMPLETION AFFIDAVIT — 2014 CURB AND SIDEWALK REPLACEMENT PROGRAM —PROJECT NO 114-013 (MVHI Mr. Gilot advised that Mr. Eric Horvath, Director of Public Works, has requested that this Change Order No. 2 (Final) and Completion Affidavit be tabled at this time. Upon a motion made by Mr. Relos, seconded by Mr. Henthom and carried, Change Order No. 2 (Final) and the Project Completion Affidavit were tabled. APPROVAL OF REOUEST TO ADVERTISE FOR THE. RECEIPT OF BIDS — REMOVAL OF — LEAD -BASED PAINT AT IVY TOWER, PHASE II — PROJECT NO. 115-030 (RIVER WEST DEVELOPMENT AREA TIF) In a memorandum to the Board, Mr. Michael Carey, Engineering, requested permission to advertise for the receipt of bids for the above referenced project. Therefore, upon a motion made by Mr. Gilot, seconded by Mr. Henthom and carried, the above request was approved. APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — HOTEL LASALLE RENOVATIONS — PROJECT NO. 115-033 (RIVER WEST DEVELOPMENT AREA TIF) In a memorandum to the Board, Mr. Michael Carey, Engineering, requested permission to advertise for the receipt of bids for the above referenced project. Upon a motion made by Mr. Gilot, seconded by Mr. Henthom and carried, the above request to advertise was approved. APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — EQUIPMENT PURCHASE FOR NELLO CORPORATION PHASE B — PROJECT NO. 115-037 (RIVER WEST DEVELOPMENT TIT) In a memorandum to the Board, Mr. Michael Carey, Engineering, requested permission to advertise for the receipt of bids for the above referenced project. Upon a motion made by Mr. Gilot, seconded by Mr. Henthom and carried, the above request to advertise was approved. APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — IN TEK SEWER _ LINE RIGHT-OF-WAY CLEANING — PROJECT NO 114-070 (SEWER OPERATIONS AND MAINTENANCE) In a memorandum to the Board, Mr. Patrick Henthom, Engineering, requested permission to advertise for the receipt of bids for the above referenced project. Upon a motion made by Mr. Gilot, seconded by Mr. Henthom and carried, the above request to advertise was. approved. APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — 2015 SEWER REHABILITATION - PROJECT NO 115-002 (SEWER OPERATIONS AND MAINTENANCE) In a memorandum to the Board, Mr. Patrick Henthorn, Engineering, requested permission to advertise for the receipt of bids for the above referenced project. Upon a motion made by Mr. Gilot, seconded by Mr. Henthorn and carried, the above request to advertise was approved. APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS FOR SALE OF ABANDONED VEHICLES (CODE ENFORCEMENT) In a memorandum to the Board, Kathy Eli, Code Enforcement, requested permission to advertise for the receipt of bids for the above referenced vehicles. Upon a motion made by Mr. Gilot, seconded by Mr. Henthorn and carried, the above request to advertise was approved.
