A coordination meeting was held with the OWNER and Memorial <br />Hospital on April 15, 2013. Based on the discussion, it was decided <br />to pursue Option # 3 with Option # 4 as an alternate. <br />The ENGINEER shall re- configure the parking lot as shown in Figure <br />6. <br />The ENGINEER shall convert the open space after the demolition of <br />the building located at 401 North Michigan Street (Figure 7) into a <br />parking area which will become part of the consolidated parking lot <br />that was constructed in 2012. <br />II. Add Section X to Appendix "A" as follows: <br />X. The ENGINEER shall provide the following services for the demolition of <br />the two buildings (see Figures 7 and 8) located at 401 North Michigan <br />Street and 502 North Michigan Street. <br />1. The ENGINEER will develop specification documents for the asbestos <br />abatement and demolition of the structure located at 401 North Michigan <br />Street and the structure located at 502 North Michigan Street, South Bend, <br />Indiana (Refer to Figure 7 & Figure 8). The OWNER and ENGINEER agree <br />that the documents will be written so that the base bid cost for the <br />demolition of above and below ground structures will be bid as a lump sum <br />item. <br />a. Site Grading and Drainage <br />The ENGINEER will include general instructions for backfill and <br />compaction, final grade and for temporary seeding. <br />b. General Site Underground Demolition <br />The ENGINEER will develop specification documents identifying, in <br />general, those materials that shall be demolished and removed or <br />potentially abandoned in place. The specification documents shall <br />also provide general information as to how the site will be prepared <br />relative to the removal of foundations and concrete slabs. <br />C. Utility Abandonment <br />The ENGINEER shall identify the general approach /guidance as how <br />utility abandonment will occur. This shall include general provisions <br />for the plugging or capping of lines to be abandoned /demolished, <br />which connect to active lines intended to remain in place throughout <br />demolition. <br />3of10 <br />Supplemental Agreement No. 3 <br />
