Pipe designated to be removed shall be clear of all debris prior to commencement of work. <br />2. Contractor shall remove buried abandoned sanitary sewer pipe within property lines. <br />J. Contractor shall construct concrete bulkheads which extend 3' into the remaining pipe along the east and west property <br />lines and shall be the full diameter of the existing pipe in order to completely plug the pipe. <br />4. Contractor shall remove manhole located within Michigan Street to 3' below grade and construct a 3' concrete bulkhead <br />to plug existing pipe. <br />5. Contractor shall break the base of the manhole in a manner that prevents entrapment of water. <br />6. Contractor shall backfill manhole with Type 5 structural fill and patch according to listed Collector Street specifications. <br />7. Concrete shall be Class A in accordance with INDOT 702.02. <br />8. Contractor shall fill pipe at east property line to manhole with non -removable Plowable fill in accordance with INDOT <br />213.07. <br />SITE PLAN CONSTRUCTION NOTES <br />1. Contractor shall obtain and comply with all required local, state and federal permits. Contractor shall comply with the <br />IDEM Construction/Land Disturbance permit that will be obtained by the Owner. <br />2. Contractor shall request existing utility location from Indiana 811 prior to commencing construction. <br />J. Damage to public and private property shall be repaired to equal or better condition at no additional cost to the Owner. <br />4. No streets shall be closed or restricted without prior approval from the local municipality. See City standards. <br />5. Contractor shall protect the work and the safety of the public and shall provide, erect and maintain barricades, signals, <br />signs and other traffic control devices in accordance with the Indiana Manual on Uniform Traffic Control Devices. <br />6. Contractor shall follow the current Indiana Department of Transportation Standard Specifications" for pavement materials <br />and installation procedures. <br />7. Contractor shall construct sidewalks, ramps, parking spaces and ADA accessible areas in accordance with the current <br />ADA standards. <br />8. Sign designations shall comply with the Indiana Department of Transportation 'Indiana Manual on Uniform Traffic Control <br />Devices" (IMUTCD). <br />9. Curb radii noted are dimensioned along the face of curb. <br />10. Contractor shall bring Drawing discrepancies and conflicts to the attention of the Engineer as soon as they are noticed, <br />for clarifications and revisions as necessary. <br />11. Contractor shall prepare Record Drawings with field locations and elevations upon completion of the work for submittal to <br />the Engineer. <br />12. All pavement removal areas to be saw cut prior to pavement removals. <br />13. All pavement patches will be to an existing longitudinal joint or all the way across the road. <br />14. Contractor shall coordinate with INDOT for permits in state Right Of Way. <br />15.
