"Will obtain access to transportation - by relocating to apartment close to bus route" - <br /> with the first portion the goal and the remainder an example of an objective. <br /> All goal statements should include specific objectives, which may be understood as "way <br /> points"in reaching the goal. <br /> Typically, objectives may be added to the Housing Plan as the participant achieves each <br /> "step", but it is also allowable for the case manager and the participant to outline all the <br /> objectives when formulating a goal. This can give a participant a "road map" to follow in <br /> achieving a mutually agreed upon goal. Goals are not realistic unless they are understood <br /> by and accepted by the ESG participant. Interventions are services or direct assistance that <br /> will facilitate the participant in reaching the goal. <br /> 3.16 — Termination of Participation & Grievance <br /> Procedure <br /> It is important that subrecipients effectively communicate termination and grievance <br /> procedures to program participants and ensure that the procedures are fully understood. <br /> Subrecipients will require each participant to sign a form indicating the participant has <br /> been provided a copy of the termination and grievance policy and an opportunity to ask <br /> questions about this policy. Posting the policy on a bulletin board in a common area within <br /> the facility is an effective way to ensure that the termination and grievance procedures are <br /> available for program participants to access at any time. <br /> A. If a participant violates ESG program requirements,the subrecipient may terminate <br /> assistance in accordance with a formal process established by the subrecipient that <br /> recognizes the rights of individuals affected. The subrecipient must exercise <br /> judgment and examine all extenuating circumstances in determining when <br /> violations warrant termination so that a program participant's assistance is <br /> terminated only in the most severe cases. <br /> B. To terminate rental assistance or housing relocation and stabilization services to a <br /> program participant,the required formal process,at a minimum, must consist of: <br /> a. Written notice to the program participant containing a clear statement of the <br /> reasons for termination; <br /> b. A review of the decision, in which the program participant is given the <br /> opportunity to present written or oral objections before a person other than <br /> the person (or a subordinate of that person) who made or approved the <br /> termination decision; and <br /> City of South Bend - ESG Policies and Procedures Manual 16 <br />
