subrecipient must re-evaluate the program participant's eligibility and the amount and <br /> types of assistance the program participant needs. <br /> Re-evaluations for homelessness prevention and rapid re-housing assistance are to be <br /> documented and saved in the client file. <br /> 3.14 - Housing Stability Case Management <br /> While providing homelessness prevention or rapid re-housing assistance to a program <br /> participant,the subrecipient must: <br /> 1. Provide the program participant with a case manager and require that the program <br /> participant to meet with a case manager not less than once per month to assist the <br /> program participant in ensuring long-term housing stability; and <br /> 2. Develop a plan to be included in the client file that will assist the program participant <br /> to retain permanent housing after the ESG assistance ends,taking into account all <br /> relevant considerations, such as the program participant's current or expected <br /> income and expenses; other public or private assistance for which the program <br /> participant will be eligible and likely to receive; and the relative affordability of <br /> available housing in the area;and <br /> 3.15 - Housing Plan <br /> The ESG program uses standardized Housing Case Management Plans that are to be <br /> saved in the client file. There are two basic forms of the Plan. The Homeless Prevention <br /> Phase Plan is intended for households which receive prevention assistance in an effort to <br /> maintain their present housing.The Rapid Re-Housing Phase Plan is aimed at households <br /> who are already experiencing homelessness. The Housing Plan must be completed and <br /> placed in the participant file. <br /> Additionally, the Plan must be used to actively assist participants in meeting established <br /> outcomes based upon individual participant need.The Plan should be referenced, revised <br /> and updated regularly throughout a participant's participation in the program. Either plan <br /> may address short term or intermediate term (up to 12 months) goals which are directly <br /> tied to the household's ability to recover and/or maintain housing stability. <br /> All goals are to be written in observable and concrete terms,e.g. <br /> "Will increase household income--through part-time employment of spouse"or; <br /> City of South Bend - ESG Policies and Procedures Manual 15 <br />
