Signed and notarized Non-Collusion Affidavit (Exhibit B) <br />h.Signed City of South Bend COVID-19 Policy (Exhibit C) <br />Program Conditions: <br />Successful applicants must agree to the following program conditions: <br />a.Radon mitigation systems must be installed according to the requirements of the current American <br />National Standards Institute/ American Association of Radon Scientists and Technologists <br />(ANSI/AARST) national consensus standard for the specific housing type to include all local and <br />State requirements. <br />b.The Respondent must be currently credentialed by the NRPP or the NRSB. <br />c.The Respondent shall provide a twelve (12) month warranty on all parts and labor on the mitigation <br />system and a five (5) year fan warranty. <br />d.All radon professionals paid through the Lead Hazard Reduction Program must be registered in the <br />System for Award Management (SAM). No contract award may be made to parties listed on the <br />government wide exclusion in SAM. <br />e.All workers doing on-site work must be a licensed radon mitigator. <br />f.Payment is contingent on the City confirming the satisfactory completion of all contracted work <br />and receipt of an invoice. <br />g.Contractor must complete the work in a timely fashion once notified of the contract award. <br />The successful applicants shall comply with the City’s ordinance and all other federal, state, and local laws and <br />regulations governing nondiscrimination in employment. <br />Registry of Pre-Qualified Lead Contractors Process: <br />1.Radon professionals must submit a completed application and all required documents to be considered for <br />addition to the Registry. <br />2.The Registry of Pre-Qualified Radon Professionals will be maintained by the Lead Grant Program Manager <br />in the Department of Community Investment (DCI). <br />3.This RFQ is a request for the submission of qualifications but is not itself an offer and shall under no <br />circumstances be construed as an offer. <br />4.DCI reserves the right to request additional information from prospective radon professionals in order to <br />fully evaluate their pre-qualification. <br />5.In order to be approved for inclusion on the registry, an individual must not be under investigation for <br />criminal behavior by any State or Federal Agency. <br />6.The City reserves the right to reject any and all applications for Pre-Qualification and refuse to include an <br />individual on the Registry if the City determines in its sole discretion that the individual lacks sufficient <br />experience, financial strength, or technical expertise to perform the work under the City’s Lead Hazard <br />Reduction Grants, or has failed to comply with applicable statutes, ordinances, rules and regulations, or has <br />failed to perform their work satisfactorily. <br />7.Radon professionals will be notified if they have been placed on the Registry following staff review of their <br />Response. <br />8.The City will issue an RFQ for qualified radon mitigators at least once every six months for the duration of
