3 <br />the lead grants. Once an individual is added to the Registry, they can remain on it for the duration of the <br />lead grant cycle. The City may reach out to the individuals, from time to time, to confirm they are still <br />interested in remaining on the Registry. <br />9.Radon professionals on the Registry must provide copies of licenses of any new employees or <br />subcontractors that are hired by the radon professional. <br />10.At any point in time following the RFQ, a radon professional may request to be placed on or deleted from <br />the Registry of Pre-Qualified Radon Professionals. To be added to the Registry, individuals must submit a <br />completed application and all supporting documents as outlined in this RFQ to the City of South Bend, <br />Department of Community Investment. <br />11.Radon professionals on the Registry will be notified of opportunities to bid on Lead Hazard Reduction <br />Program work. It is the responsibility of the individual to submit a complete bid by the established deadline, <br />in order to be considered for the award of a contract. Work will be awarded to the lowest, responsive and <br />responsible bid. <br />12.The City reserves the right to remove radon professionals from the Registry based on non-responsiveness, <br />quality of work, repeated failures of the installed mitigation systems that bring level below the EPA <br />threshold or failure to provide updated information as reasonably requested by the City.
