On the tape <br />sealing the container, the collection site person shall sign and enter <br />the date specimens were sealed in the containers for shipment. The <br />collection site person shall ensure that the chain of custody <br />documentation is enclosed in each container sealed for shipment to <br />the drug testing laboratory. <br />Since specimens and documentation are sealed in shipping containers <br />that would indicate any tampering during transit and couriers, express <br />carriers, etc. do not have access to the chain of custody forms, this <br />Policy does not require that such personnel document chain of custody <br />for the shipping container during transit. This Policy also does not <br />require that there be a chain of custody entry when a specimen sealed <br />in a shipping container is put into or taken out of secure storage at the <br />collection site prior to pickup by such personnel. This means that the <br />chain of custody is not broken, and a test will not be canceled, merely <br />because such individuals have not documented their participation in <br />the chain of custody or because the chain of custody does not contain <br />entries related to putting the specimen into or removing it from secure <br />temporary storage at the collection site. <br />Failure To Cooperate <br />Any employee required to provide a urine sample will be expected to <br />sign a consent or release form authorizing the collection of the <br />specimen, analysis of the specimen for designated controlled <br />substances, and release of the results to the employer. The employee <br />will not be required to waive liability with respect to negligence on the <br />part of any person participating in the collection, handling or analysis <br />of the specimen or to indemnify any person for the negligence of <br />others. <br />lf the employee refuses to cooperate during the collection process <br />(e.9., refusal to provide a complete specimen, complete paperwork, <br />initial specimen), the collection site person will inform Visu-Sewer's <br />representative and document the employee's conduct on the Urine <br />Custody and Control Form. Employees are expected to exercise good <br />faith and cooperate during the collection process and failure to do so <br />will subject the employee to disciplinary action, up to and including <br />discharge, independent and regardless of the results of any <br />subsequent drug test. <br />Controlled Substance Test Results <br />All confirmed positive and negative test results will be reported by the <br />laboratory to a medical review officer (MRO) before they are <br />communicated to Visu-Sewer.
