Collection of Specimens <br />Visu-Sewer respects its prospective employees and employees and therefore subscribes <br />to the collection protocol set forth by the Department of Health and Human Services <br />(DHHS) which protects the privacy and confidentiality of the donor. This protocol allows for <br />the submission of the specimen under closely structured conditions but behind either a <br />closed door or a privacy partition. lt must be understood, however, that under <br />circumstances established by DHHS, the donor may be asked to provide fresh specimen in <br />the presence of a witness if the collector has reason to believe: (1) that the specimen is not <br />that of the donor; (2) that the specimen has been adulterated or altered; (3) that the <br />collection is part of a post-treatment monitoring program; or (4) that the donor has been <br />known or suspected of adulterating previous specimen(s). All specimens are collected and <br />processed by social security number and accession number for complete confidentiality. <br />Specimens to be tested will be urine in the case of a drug test and breath in the case of a <br />test for alcohol. <br />Gustody and Control Form <br />All specimens will be processed and monitored by way of an approved custody and control <br />form which is used to track the specimen from point of submission to point of destruction. <br />Employees will be required to sign the chain of custody form. Failure to do so will result in <br />the need for a retest. However, an employee or perspective employee who refuses to sign <br />after being requested to do so will be considered to have refused to submit to testing. lf <br />submission testing is refused, the prospective employee will be denied employment or in <br />the case of the employee, be subject to immediate termination of employment. <br />Laboratory Aspects of Testing <br />. Controlled Substances <br />ln order to assure our prospective employees and employees the greatest accuracy <br />and confidentiality, all drug testing will be conducted by a laboratory certified by <br />Department of Health and Human Service (DHHS) and that participates in one of <br />the following programs: guidelines of the Wisconsin State Lab of Hygiene, a <br />laboratory certified by the National lnstitute on Drug Abuse (NIDA) meeting <br />mandatory federal guidelines, a laboratory accredited by the College of American <br />Pathologists in Northfield, lllinois, or a laboratory licensed to test for drugs by the <br />New York State Department of Health, and will be conducted in two parts: <br />Screening for five classes of drugs: amphetamines, benzoylecognine <br />(cocaine metabolites), opiates, PCP, THC (marijuana metabolites); and <br />Confirmation by gas chromatography/mass spectrometry (GC/MS). <br />Note: Under Minnesota Statute (NonDot), the drugs most commonly tested <br />for include: Alcohol, Amphetamines, Barbiturates (Phenobarbital, <br />Secobarbital, etc.), Benzodiazepines (Vallium, etc.), Cocaine, Opiates <br />(codeine, morphine, heroin), PCP (Phencyclidine or "Angel Dust") THC <br />(metabolites Marijuana). <br />a <br />a <br />9
