a <br />unusual behavior and, if necessary, knowledge of use of any substances. <br />The co-employee's identity and participation in the investigation will be kept <br />confidential. Any information obtained in the investigation will be documented <br />and considered confidential. The employee's privacy will be protected as <br />much as possible. <br />Follow-up <br />Any employee who is required to submit or voluntarily submits to drug andior alcohol <br />counseling or a rehabilitation program may not return to work until he/she has been <br />evaluated by a professional in substance abuse treatment; has followed the <br />recommendation of the professional in substance abuse treatment, including testing; <br />has been certified by the professional in substance abuse treatment or the <br />Company's medical review officer (MRO)as drug and alcohol free; and has <br />successfully passed a drug and alcohol test by receiving a negative test result. <br />Employees returning to work will be subject to afollow-upG testing consisting of <br />periodic, unannounced testing as the Company may require to ensure that the <br />employee is continuing to refrain from using drugs and/or alcohol or as the <br />rehabilitation counselor may direct as part of the employee's treatment for a period <br />of at least 12-months following completion of the program. <br />Collection Site <br />The collection site is a medical facility or other designated suitable place which may include <br />a mobile facility where individuals present themselves for the purpose of drug or alcohol <br />testing. The site will have available all the necessary equipment, personnel and materials <br />for testing. The site will afford visual and aural privacy to the individual being tested and <br />security sufficient to prevent unauthorized persons from tampering with specimens <br />collected. ln the case of breath alcohol testing, the site should provide security sufficient to <br />prevent unauthorized persons from seeing or hearing test results (APPENDIX F - <br />AUTHORTZED MEDTCAL FAC\ LITIES/COLLECT\ ON S/rES). <br />Specimen Gollection for Drug and Alcohol Tests ,3A2.001(b)6 <br />The specimen collection for drug and alcohol tests will be at a collection site approved <br />andior designated by Visu-Sewer. Except in emergency situations, appointments for <br />specimen collection will be scheduled by Visu-Sewer for the employee during working <br />hours at the convenience of Visu-Sewer. ln emergency (accident or injury) situations, the <br />employee should report to the nearest specimen collection facility to where the emergency <br />occurred. The employee will be required to present positive identification (e.g., photo l.D. <br />card) at the time of testing. Visu-Sewer will provide instructions to the collection facility <br />stating the requirements for specimen collection (APPENDIX H - POSI- <br />ACCIDENT/INJURY /NSTRUCflONS, APPENDIX I - AUTHORIZATION FOR POSr- <br />AC C I D E NT/ t N J U RY C O N T RO LLED S UBS TA N C E S/A LC O H O L rES I/NG ). Testi n g <br />procedures will be explained to the employee (APPENDIX J -SPECIMEN COLLECTION <br />PROCEDURE). The employee will be compensated for the time necessary to travel to and <br />from the collection site and the length of time required to perform the collection process. <br />8
