require the following information: <br /> (1) The business name and business telephone number of <br /> the commercial business making application. <br /> (2 ) Whether the applicant is an individual, <br /> partnership or corporation. If an individual, the <br /> name and business and residence address of the <br /> applicant shall be given; if a partnership, the <br /> name and business and residence address of each <br /> partner shall be given; if a corporation, the <br /> name, date and state under which such corporation <br /> was organized, and the name and business address, <br /> resident agent, and a copy of the Certificate of <br /> Incorporation. <br /> (3 ) Number of vehicles which are specifically designed <br /> to transport mobility handicapped individuals <br /> which are owned and/or leased by the applicant; <br /> listed by year, make, license plate number of <br /> vehicle, and commercial marking on vehicles which <br /> would display the permit. <br /> (4 ) Written Proof of Liability Insurance. <br /> (5 ) An annual fee of twenty-five ( $25. 00 ) dollars. <br /> (43)- (d) Within thirty ( 30 ) days of filing a complete <br /> application form with the Board of Public Works, the Board shall <br /> instruct the City Controller 's office to issue a South Bend <br /> Handicapped Parking Permit to the applicant if it is found that the <br /> material facts of the application are true. In the case of <br /> commercial applications, South Bend Handicapped Parking Permits shall <br /> be issued to applicants conforming with the provisions of this <br /> Article, and shall be issued in the following number: <br /> Number of Vehicles Specially <br /> Designed to Transport Handicapped Persons Maximum <br /> per Business Permits Allowed <br /> 1 1 <br /> 2 <br /> 3 3 <br /> 4 <br /> 5 5 <br /> Over 5 6 <br /> -( )- (e) Upon the issuance of such permit by the City <br /> Controller' s office, the permit shall be displayed in the front <br /> windshield of the driver' s side, by the permit holder. <br /> (f ) Commercial businesses issued permits under this Article <br /> shall be permitted to use designated parking places only when in the <br /> process of loading or unloading mobility handicapped individuals. <br /> -(-e-)- (g) Any permit issued pursuant to this Article shall <br /> expire on December 31st of the year issued. <br /> SECTION II. This Ordinance shall be in full force and effect <br /> from and after its passage by the Common Council and approval of the <br /> Mayor, and legal publication. <br /> A A/7 4 4 <br />
