<br />Preventive Maintenance of Angela Boulevard Bridge <br />Over St. Joseph River January 2023 <br /> 9 <br /> <br />2. The Contractor shall employ a qualified third party, professional testing <br />laboratory to provide all material testing, approval testing and system testing. <br />There will be no testing performed by the St. Joseph County Engineer’s staff. <br />The tests to be provided and the frequency of testing shall be as required for <br />the INDOT Standard Specifications for highway materials and approvals. <br /> <br />3. Two copies of all test reports are to be submitted to the Engineer. All reports <br />are to be certified by a Professional Engineer registered in the State of <br />Indiana. <br /> <br />4. If a supplier of materials has a current immediate usage approval by INDOT, <br />compliant material will not require further testing. <br /> <br />5. Material testing will not be paid for separately and shall be considered <br />incidental to the contract work. Cost for testing shall be included in each <br />respective pay item. <br /> <br />XVIII. SAFETY <br /> <br />A. 2022 INDOT Standard Specification: 107.08 <br /> <br />B. Additions: <br /> <br />1. The IOSHA Regulation 29 C.F.R. 1926, Subpart P, for all construction <br />excavations and trench safety systems is hereby incorporated into these <br />Contract Documents by reference. The cost of the safety systems shall not <br />be paid for separately; but shall rather be included in the unit prices bid for <br />the work. <br /> <br />2. Entry into all confined facilities shall be in accordance with Local, State, and <br />Federal Confined Space Entry Laws and Regulations. <br /> <br />XIX. RETAINAGE <br /> <br />A. 2022 INDOT Standard Specification: 108 and 109 <br /> <br />B. Additions: <br /> <br />1. Retainage: <br /> <br />a. The County will retain 5% of each monthly payment earned. <br /> <br />b. The County will retain 5% of all Change Order work. <br /> <br />XX. MOBILIZATION AND DEMOBILIZATION <br /> <br />A. 2022 INDOT Standard Specification: 110 <br /> <br />B. Additions: <br /> <br />1. The lump sum unit price for this item shall not be more than five (5%) <br />percent of the total of all other bid items of the original proposal.
