<br />Ms. Brach recommended that the Board award the contract to the lowest responsive and
<br />responsible quoter, Martell Electric LLC, 4601 Cleveland Rd., South Bend, IN 46628, in the
<br />amount of $31,512. Therefore, Mr. Molnar made a motion that the recommendation be accepted,
<br />and the quotation be awarded, and the contract approved as outlined above. Mr. Gathers
<br />seconded the motion, which carried by roll call.

<br />APPROVE CHANGE ORDER NO. 1 – LINCOLN WAY WEST AND OLIVE STREETSCAPE
<br />– PROJECT NO. 119-108 (PO-0009634)
<br />Ms. Maradik advised that Mr. Finnian Cavanaugh, Engineering, submitted Change Order No. 1
<br />on behalf of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, IN 46614
<br />indicating the contract amount be increased by $55,639.58 for a new contract sum, including this
<br />Change Order, in an amount not to exceed $1,796,510.78. Upon a motion made by Mr. Molnar,
<br />seconded by Mr. Gathers and carried by roll call, the Change Order was approved.

<br />APPROVE CHANGE ORDER NO. 1 – LIBERTY TOWER EXTERIOR RENOVATIONS,
<br />PHASE II – PROJECT NO. 120-047 (PO-0006614)
<br />Ms. Maradik advised that Mr. Kyle Silveus, Engineering, submitted Change Order No. 1 on
<br />behalf of Slatile Roofing & Sheet Metal Co., Inc., 1703 S. Ironwood Dr., South Bend, IN 46613,
<br />indicating the contract amount be increased by $2,882 for a new contract sum, including this
<br />Change Order, in the amount of $121,100. Upon a motion made by Mr. Molnar, seconded by Mr.
<br />Gathers and carried by roll call, the Change Order was approved.

<br />APPROVE CHANGE ORDER NO. 1 – 2021 CRACK SEALING – PROJECT NO. 121-020
<br />(PO-0010107)
<br />Ms. Maradik advised that Mr. Finnian Cavanaugh, Engineering, submitted Change Order No. 1
<br />on behalf of Asphalt Restoration Services, LLC, PO Box 2484, South Bend, IN 46680,
<br />indicating the contract amount be increased by thirty-five (35) days for a new completion date,
<br />including this Change Order, of October 1, 2021. Upon a motion made by Mr. Molnar, seconded
<br />by Mr. Gathers and carried by roll call, the Change Order was approved.

<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />– CSO 35 THROTTLE PIPE REPLACEMENT – PROJECT NO. 120-042 (PO-0006388)
<br />Ms. Maradik advised that Mr. Jacob Klosinski, Engineering, submitted Change Order No. 1
<br />(Final) on behalf of Selge Construction Co., Inc., 2833 S. 11th. St., Niles, MI 49120, indicating
<br />the contract amount be decreased by $14,057.50 for a new contract sum, including this Change
<br />Order, of $134,730. Also submitted was the Project Completion Affidavit indicating this new
<br />final cost of $134,730. Upon a motion made by Mr. Molnar, seconded by Mr. Gathers and
<br />carried by roll call, Change Order No. 1 (Final) and the Project Completion Affidavit were
<br />approved.

<br />APPROVE TITLE SHEET– AIRPORT ANNEXATION AREA WATER MAIN EXTENSION
<br />– PROJECT NO. 120-026
<br />Mr. Jacob Klosinski, Engineering, advised that the Title Sheet for the above referenced project
<br />was being presented at this time for execution. Upon a motion made by Mr. Molnar, seconded
<br />by Mr. Gathers and carried by roll call, the above referenced Title Sheet was approved and
<br />signed.

<br />APPROVE TITLE SHEET– COAL LINE TRAIL, PHASE II – PROJECT NO. 1401811
<br />Mr. Kyle Silveus, Engineering, advised that the Title Sheet for the above referenced project was
<br />being presented at this time for execution. Ms. Maradik noted the project would be bid out by
<br />INDOT with an anticipated timeframe of January 2022. Upon a motion made by Mr. Molnar,
<br />seconded by Mr. Gathers and carried by roll call, the above referenced Title Sheet was approved
<br />and signed.

<br />APPROVE TITLE SHEET– NORTH PUMPING STATION FLOOD DAMAGE REPAIRS –
<br />PROJECT NO. 118-302R
<br />Mr. Jacob Klosinski, Engineering, advised that the Title Sheet for the above referenced project
<br />was being presented at this time for execution. Upon a motion made by Mr. Molnar, seconded
<br />by Mr. Gathers and carried by roll call, the above referenced Title Sheet was approved and
<br />signed.
