<br />Page 10 of 42 <br />i. INDOT shall notify the CITY in writing of any addition or deletion of traffic <br />signal installations to Exhibit A, and the effective date of the addition or <br />deletion. <br />ii. Within the City of South Bend, and surrounding areas, all new traffic signals, <br />which are to be installed by private parties on the State highway system, shall <br />be submitted to the CITY for their information prior to INDOT granting <br />permission for such installation. <br />iii. INDOT shall coordinate with the CITY on the design of new signals or the <br />modernization of existing signals on the State highway system within St. <br />Joseph County to ensure that the design is compatible with the CITY’s traffic <br />control system. <br />iv. The CITY shall be invited to the pre-construction conference and the final <br />inspection of newly installed or modernized traffic signals for which the CITY <br />shall provide maintenance under this Agreement. <br />v. The CITY shall accept a new signal installation for maintenance after INDOT <br />has furnished the CITY a copy of its Form I.C. 636A (Completion Report) for <br />installation indicating that the new signal is installed in accordance with <br />INDOT “Standards and Specifications” and is approved by both parties as an <br />addition to this Agreement. <br />vi. As long as a highway remains under the jurisdiction of INDOT, if the control, <br />operation, or maintenance of a traffic signal on a special signal system is not <br />satisfactory to INDOT, said signal will be removed from the special signal <br />system by the CITY and placed on a separate system controlled by INDOT. <br />C. The maintenance card in the controller and/or flasher cabinet shall have an entry made <br />for every activity performed at an intersection by the CITY. Entries on the maintenance <br />card shall include the date, the initials of the technician and a brief description of the <br />work performed. <br />D. No change will be made to any traffic signal which will alter its design, phasing, timing <br />or functional operations without authorization from the INDOT LaPorte District <br />Office. <br />E. INDOT reserves the right to enter at any time onto any signalized intersection covered <br />under this Agreement or into any controller cabinet for inspection, alteration or change
