the manufacturer's current technical data. Materials used in the construction shall be <br />new and not less than the quality conforming to current engineering and <br />manufacturing practices. Materials shall be free of defects and shall be suitable for <br />the intended use. <br /> <br />Any exceptions to these specifications must be clearly pointed out. Otherwise, it will <br />be considered that the items offered are in strict compliance with the written <br />specifications and that the successful bidder will be responsible for delivering a <br />vehicle meeting these specifications. Any exceptions must be marked as such within <br />the body of the bid and explained on a separate page marked "EXCEPTIONS". <br /> <br />Bids taking total exception to these specifications will not be accepted. <br /> Y___N___ <br /> <br />1.1 INFORMATION AND DESCRIPTIVE MATERIAL <br /> <br />The bidder must furnish all information requested in the space provided on the bid <br />form. The manufacturer must supply a dated copy of their Ford QVM certificate as <br />proof that they are a current member of the Qualified Vehicle Modifier Program. This <br />requirement must be met even if the chassis specified is not a Ford to ensure that <br />chassis modification standards meet a desired minimum level. The manufacturer <br />must also supply the D-spec certification for this particular type, at least one [1] <br />complete set of sketches, descriptive literature, and complete specifications covering <br />the products offered. Bids not meeting this requirement will be considered non- <br />responsive and rejected. <br /> Y___N___ <br /> <br />1.2 PRICES AND PAYMENTS <br /> <br />All bid prices shall be on a F.O.B. destination and acceptance basis at the <br />Purchaser's specified location. These Prices shall be complete and include <br />warranty. Payment shall be made in accordance with these specifications and the <br />Bid Proposal submitted by the Bidder. Payment shall be made upon acceptance of <br />the vehicle(s) and equipment specified under these specifications. All bid prices and <br />conditions must be specified on the bid proposal form. Bid prices shall be valid for at <br />least thirty- [30] days from the date of the bid opening or as otherwise specified on <br />the bid proposal form. Full payment shall be made as each unit is received, <br />inspected, and found to comply with the procurement specifications. <br /> Y___N___ <br /> <br />1.2.1 Price Guarantee <br /> <br />The successful bidder shall guarantee the bid price for a minimum of 12 months to <br />the City of South Bend. The City reserves the right to purchase an additional unit at <br />this price within 12 months of bid award. <br /> <br />1.3 GENERAL LIABILITY <br /> <br />Bidders are required to submit with their bids a Certificate of Liability Insurance in the <br />amount of five million dollars (5,000,000) in general liability as issued by the bidder’s <br />insurance company. <br /> Y___N___ <br />1.4 WARRANTY <br /> <br />The successful manufacturer shall provide a three (3) year/36,000 mile warranty on
