<br />APPROVE CHANGE ORDER NO. 2 — GEMINI AT COLFAX EXTERIOR RENOVATIONS,
<br />DIVISION B — PROJECT NO. 119-103 (PO-0001015)
<br />Mr. Gilot advised that Mr. Zach Hurst, Engineering, has submitted Change Order No. 2 on
<br />behalf of Slatile Roofing and Sheet Metal Co., Inc., 1703 S. Ironwood Dr., South Bend, IN
<br />46613, indicating the contract amount be increased by $22,443 and an additional twelve (12)
<br />days for a new contract sum, including this Change Order, in the amount of $368,128 and a new
<br />completion date of September 4, 2020. Ms. Maradik noted the Change Order exceeds the legal
<br />20% limit; however, this was due solely to unforeseen conditions. Upon a motion made by Ms.
<br />Maradik, seconded by Mr. Molnar and carried by roll call, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— 2019 COMMUNITY CROSSINGS ROUND 2 — PROJECT NO. 119-065 (PO-0000237)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, IN 46614,
<br />indicating the contract amount be decreased by $7,467.97 for a new contract sum, including this
<br />Change Order, of $773,982.03. Also submitted was the Project Completion Affidavit indicating
<br />this new final cost of $773,982.03. Upon a motion made by Ms. Maradik, seconded by Mr.
<br />Molnar and carried by roll call, Change Order No. 1 (Final) and the Project Completion Affidavit
<br />were approved.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— TUTT STREET BRICK RECOVERY — PROJECT NO. 120-045 (PO-0003334)
<br />Mr. Gilot advised that Mr. Zach Hurst, Engineering, has submitted Change Order No. 1 (Final)
<br />on behalf of Walsh & Kelly, Inc., 24358 State Road 23, South Bend, IN 46614, indicating the
<br />contract amount be increased by $2,894 for a new contract sum, including this Change Order, of
<br />$23,889. Also submitted was the Project Completion Affidavit indicating this new final cost of
<br />$23,889. Upon a motion made by Ms. Maradik, seconded by Mr. Molnar and carried by roll call,
<br />Change Order No. 1 (Final) and the Project Completion Affidavit were approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — CENTURY CENTER PARKING LOT
<br />ENTRY GATEWAY IMPROVEMENTS — PROJECT NO. 118-035A (HOTEL/MOTEL TAX
<br />FUND, TRAFFIC SIGNAL MAINTENANCE)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Larson -Danielson Construction Co., Inc., 302 Tyler St., LaPorte, IN
<br />46350, for the above referenced project, indicating a final cost of $158,724.43. Upon a motion
<br />made by Ms. Maradik, seconded by Mr. Molnar and carried by roll call, the Project Completion
<br />Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — PINHOOK PARK PAVILION
<br />IMPROVEMENTS — PROJECT NO. 118-077B (PARK BOND H & J, DNR GRANT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Larson -Danielson Construction Co., Inc., 302 Tyler St., LaPorte, IN
<br />46350, for the above referenced project, indicating a final cost of $185,043.23. Upon a motion
<br />made by Ms. Maradik, seconded by Mr. Molnar and carried by roll call, the Project Completion
<br />Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — PULASKI PARK IMPROVEMENTS —
<br />PROJECT NO. 118-056 (PO-0001782)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Larson -Danielson Construction Co., Inc., 302 Tyler St., LaPorte, IN
<br />46350, for the above referenced project, indicating a final cost of $898,527.75. Upon a motion
<br />made by Ms. Maradik, seconded by Mr. Molnar and carried by roll call, the Project Completion
<br />Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — GEMINI AT COLFAX EXTERIOR
<br />RENOVATIONS, DIVISION C — PROJECT NO. 119-103 (RWDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Paul's Welding, Inc., 7930 W. 1000 N., Nappanee, IN 46550, for the
<br />above referenced project, indicating a final cost of $163,000. Upon a motion made by Ms.
<br />Maradik, seconded by Mr. Molnar and carried by roll call, the Project Completion Affidavit was
<br />approved.
