<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT

<br />SEWAGE WORKS CAPITAL, WASTEWATER TREATMENT & DISPOSAL EQUIP./2020
<br />SEWAGE WORKS, OPERATIONS, OTHER EQUIP. REPAIR & MAINT )
<br />Mr. Jacob Klosinski, Engineering, submitted Change Order No. 1 (Final) on behalf of
<br />Cornerstone Stewards, Inc., 5720 Huguenard Rd., Fort Wayne, IN 46818, indicating the contract
<br />amount be increased by $10,833 for a new contract sum, including this Change Order, of
<br />$380,833. Also submitted was the Project Completion Affidavit indicating this new final cost of
<br />$380,833. Upon a motion made by Ms. Dorau, seconded by Mr. Molnar and carried by Mr.
<br />Gilot, Ms. Maradik, and Mr. Gathers, Change Order No. I (Final) and the Project Completion
<br />Affidavit were approved. _
<br />APPROVE CHANGE ORDER NO. 6 (FINAL_) AND PROJECT COMPLETION AFFIDAVIT
<br />— LIBERTY TOWER EXTERIOR RENOVATIONS — PROJECT NO. 118-086R (RWDA TIF)
<br />Mr. Toy Villa, Engineering, submitted Change Order No. 6 (Final) on behalf of Environmental
<br />Glass, Inc., 12675 Marlin Dr., Redford, MI 48239, indicating the contract amount be decreased
<br />by $5,000 for a new contract sum, including this Change Order, of $306,817.08. Also submitted
<br />was the Project Completion Affidavit indicating this new final cost of $306,817.08. Upon a
<br />motion made by Ms. Dorau, seconded by Mr. Molnar and carried by Mr. Gilot, Ms. Maradik,
<br />and Mr. Gathers, Change Order No. 6 (Final) and the Project Completion Affidavit were
<br />approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — VESTED INTEREST BUILDING
<br />ROOF REPAIR — PROJECT NO. 119-049 (RWDA TIF)
<br />Mr. Toy Villa, Engineering, submitted the Project Completion Affidavit on behalf of Midland
<br />Engineering Company, Inc., 52369 State Road 933 North, South Bend, IN 46637, for the above
<br />referenced project, indicating a final cost of $248,088.10. Upon a motion made by Ms. Dorau,
<br />seconded by Mr. Molnar and carried by Mr. Gilot, Ms. Maradik, and Mr. Gathers, the Project
<br />Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — BENDIX THEATER FLOORING —
<br />PROJECT NO. 119-049 (HOTEL MOTEL TAX)
<br />Mr. Toy Villa, Engineering, submitted the Project Completion Affidavit on behalf of Carpet
<br />Service Outlet, Inc., 1514 Mishawaka Ave., South Bend, IN 46615, for the above referenced
<br />project, indicating a final cost of $72,642. Upon a motion made by Ms. Dorau, seconded by Mr.
<br />Molnar and carried by Mr. Gilot, Ms. Maradik, and Mr. Gathers, the Project Completion
<br />Affidavit was approved.
<br />APPROVE REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS AND TITLE SHEET —
<br />WASHINGTON STREET IMPROVEMENT PROJECT — PROJECT NO. 118-105 (COIT)
<br />In a memorandum to the Board, Mr. Kyle Silveus, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Also presented at this time for
<br />approval and execution was the Title Sheet. Upon a motion made by Ms. Dorau, seconded by
<br />Mr. Molnar and carried by Mr. Gilot, Ms. Maradik, and Mr. Gathers, the above request to
<br />advertise was approved, and the Title Sheet was approved and signed.
<br />APPROVE REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — 2020 STREET AND
<br />SEWER DEPARTMENT MATERIALS — PROJECT NO. 120-005 (STREETS, CURB &
<br />SIDEWALK, SEWER CONCRETE, SEWER INS., WATER WORKS, PARKS, MVH-
<br />RESTRICTED)
<br />In a memorandum to the Board, Mr. Scott Kreeger, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Therefore, upon a motion made
<br />by Ms. Dorau, seconded by Mr. Molnar and carried by Mr. Gilot, Ms. Maradik, and Mr. Gathers,
<br />the above request was approved.

<br />and carried by Mr. Gilot, Ms. Maradik, and Mr. Gathers, the above request to advertise was
<br />approved, and the Title Sheet was approved and signed.
