Version 6-8-2017 <br />Deliverables - Upon completion and final approval of the work by INDOT, the CONSULTANT shall deliver <br />to the LPA and INDOT Project Manager the following in accordance with INDOT policies and procedures: <br /> <br />1 set of final approved tracings and one (1) set of prints with cross-sections of the contract plans <br />drawn to a suitable scale on standard 34" x 22" sheets prepared with the following process: Ink on <br />approved ink medium or computer plotter for all sheets, except cross-sections for computing <br />earthwork quantities. The cross-sections for computing earthwork quantities shall be on approved <br />cross-section paper, in ink, or as plotted by computer plotter. <br /> <br />2 hard copies and an electronic copy of special provisions for the specifications using INDOT's <br />current word processing software. <br /> <br />2 hard copies and an electronic copy of the construction cost estimate in current INDOT approved <br />format. <br /> <br />1 hard copy and an electronic copy of the estimate of quantities in current INDOT approved format. <br /> <br />2 copies of all quantity computations, including screed elevations. <br /> <br />1 copy of all design computations, indexed, paged and bound. <br /> <br />1 completed Scope/Environmental Compliance Certification/Permit Application Certification Form. <br /> <br />1 completed right-of-way certification. <br /> <br />1 completed LPA railroad certification. <br /> <br />Task 3 Utility Coordination Services <br /> <br />The CONSULTANT shall conduct office reviews, field inspections, investigations, and meetings as <br />needed for utility relocations. The CONSULTANT shall provide legal notice before entering private property. <br /> <br />The CONSULTANT shall prepare agreements and authorization letters for utility relocations. <br /> <br />After Design Approval, the CONSULTANT shall distribute plans with a tentative project schedule to the <br />utility companies. Also, the CONSULTANT shall request submittal of relocation plans with an estimated <br />relocation schedule. All plan revisions affecting utilities must be sent to the affected utilities. <br /> <br />The CONSULTANT shall obtain written documentation from all utility companies whether relocations <br />are required or not, unless instructed otherwise by INDOT's Highway Utility Engineer. <br /> <br />The CONSULTANT shall review relocation plans and schedules to verify that all conflicts are resolved. <br />At the appropriate time, INDOT will provide written notice for the utility company to proceed. <br /> <br />The CONSULTANT shall distribute two copies of the utility written notice to proceed with approved <br />utility relocation plans to the INDOT District. <br /> <br /> A-4 <br /> <br />
