Mr. <br />Harcus continues... <br />We <br />request that the Commissioners approve the attached re- <br />lated <br />Change Order No. 5 which increases cost by $120.00, <br />Ms <br />Auburn made a motion to approve Change Order #5, with <br />R. <br />A. Stickler Plumbing, for an increase in contract price <br />of <br />$120.00, at the address of 1111 E. Miner Street, seconded <br />by <br />Mr. Robinson, and the motion was unanimously carried. <br />g. Conmission <br />approval requested for Change Order #3, with <br />CHANGE ORDER #3 <br />Plaia <br />Construction Company, for an amended contract total <br />WITH PLAIA CON - <br />of <br />1,088.00, at the address of 1237 E. Miner Street <br />STRUCTION COMPANY <br />So <br />th Bend, Indiana, as set forth in letter from Mr. <br />APPROVED. <br />James <br />Harcus, Director, Bureau of Housing. <br />Mr. <br />Harcus: It was discovered that the upper cement step <br />of <br />the set specified to be replaced at 1237 E. Miner was <br />actually <br />lodged in the structure foundation. We hesitate <br />to <br />disturb this arrangement. The Commissioners are re- <br />qu <br />sted to approve the attached Change'Order No. 3 which <br />prescribes <br />replacement of only the hazardous bottom step <br />anc <br />repair of some badly damaged cement walkway leading <br />to <br />steps. No change in contract price would result. <br />Mr. <br />Robinson made a motion to approve Change Order #3, <br />with <br />Plaia Construction Company, for no change in con- <br />tract <br />price, at the address of 1237 E. Miner Street, <br />seconded <br />by Ms. Auburn,,and the motion was unanimously <br />carried. <br />h. Co <br />fission approval requested for Change Order #5, with <br />CHANGE ORDER #5 <br />Pl <br />ia`Construction Company, for an increase in contract <br />WITH PLAIA CON - <br />price <br />of $147.00, for an amended contract total of <br />STRUCTION COMPANY <br />$4,247.00, <br />at the address of 1111 East Sorin, South <br />APPROVED <br />Bend, <br />Indiana, as set forth in letter from Mr. James <br />Harcus, <br />Director, Bureau of Housing. <br />Mr. <br />Harcus: While replacing bath tub at 1111 E. Sorin, <br />sutifloor <br />and joists fell through with contractor. The <br />Commissioners <br />are requested to approve <br />Change <br />Order No. 5 which prescribes joists, subfloor <br />replacement <br />and a new heat run necessitated by water <br />cl <br />set installation (Change Order No. 4). This increases <br />co <br />t by $147.00. <br />Mr. Robinson made a motion to approve Change Order #5, <br />with Plaia Construction Company, for an increase in <br />contract price of $147.00, at the address of 1111 East <br />So in, seconded by Mr. Cira, and the motion was unani- <br />mo sly carried. <br />-15- <br />
