At the time <br />of the inspection, the homeowner was using the window for an <br />air conditioner but has since discontinued its use. <br />Mr. Richard E. Pfeifer, Chief Inspector, Northwest Bureau of <br />Housing, in his memorandum of October 4, 1976, was recommend- <br />ing this request be approved. The dwelling is at 752 North <br />CHANGE <br />Cleveland Avenue, and increases the contract amount from <br />ORDER <br />$6,342.00 to an amended contract total of: $6,374.00. <br />NO. 1 TO <br />On motion by Mr. Wiggins, seconded by Mr. Cira and carried, <br />CONTRACT <br />the above Change Order was approved. <br />NWCD /PR/ <br />Change Order No. 2 to Contract NWCD /PR /CS- 1/05: This is for <br />CHANGE <br />an increase in contract amount of $824.00, for work on the <br />ORDER <br />front porch, 6' x 19', to include tearing out the porch deck- <br />NO. 2 TO <br />ing, joists and columns. Install 2' x 6' joists 16" O.C. and <br />CONTRACT <br />tongue and groove decking, paint decking two coats. Install <br />NWCD /PR/ <br />28 ft. of lattice work and paint two coats. Install two wrought <br />CS -1/05 <br />iron corner posts and two wrought iron flat posts. <br />APPROVED, <br />NWCD <br />Mr. Brownell read Mr. Pfeifer's memorandum which stated the <br />HOUSING <br />porch decking on the change order was detected and reported in <br />PROGRAM <br />the specification write up. However, the Northwest Advisory <br />Board requested to have it deleted from the contract. Since the <br />time of the initial inspection, this porch deck has deteriorated <br />to the point of being unsafe for the occupants. Therefore, a <br />telephone poll of the Advisory Board members was made and the <br />entire Advisory Board voted "yes" to the replacements specified <br />on the change order. It is recommended that the contract be <br />amended to include the items on the Change Order and that it <br />be approved. <br />The dwelling is at 537 North Euclid Avenue, and increases the <br />contract amount from $2,156.00 to an amended contract total of: <br />$3,599.00. <br />On motion by Mr. Wiggins, seconded by Mr. Cira and carried, <br />the above Change Order was approved. <br />31 Change Order No. 1 to Contract NWCD /PR /CS -1/04: This is for <br />CHANGE <br />specification changes with contract price unchanged for dwell- <br />ORDER <br />ing at 612 North Walnut Street. Contract amount: $3,207.00. <br />NO. 1 TO <br />CONTRACT <br />Mr. Pfeifer, in his memorandum of October 11, 1976, advised <br />NWCD /PR/ <br />since the time of the initial inspection, the homeowner has <br />CS -1/04 <br />replaced the sprayer on the sink. However, she has leak <br />APPROVED, <br />problems with her fixtures in the kitchen and bathroom. The <br />NWCD <br />contractor was instructed to submit a change order for the <br />HOUSING <br />deletion of the sprayer and the additions requested on the <br />PROGRAM <br />change order at no additional cost. <br />
