<br />AWARD PROPOSAL — CITY-WIDE OVERHEAD DOOR SERVICES (VARIOUS
<br />DEPARTMENT'S MAINTENANCE FUNDS)
<br />Mr. Adam Parsons, Central Services, advised the Board that on November 26, 2019, proposals
<br />were received and opened for the above referenced proposal. After reviewing those proposals, Mr.
<br />Parsons recommended that the Board award the contract to the highest scoring vendor, Industrial
<br />Door of Northern Indiana, 3839 S. Main St., South Bend, IN 46614. Therefore, Ms. Maradik made
<br />a motion that the recommendation be accepted, and the proposal be awarded as outlined above.
<br />Ms. O'Sullivan seconded the motion, which carried.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) — 2019 TREE PLANTINGS — PROJECT NO
<br />119-013 (MVH)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of Douglas Landscape, Inc., 23590 Ardmore Trail, South Bend, IN 46628, indicating the
<br />contract amount be decreased by $3,690 for a modified contract sum, including this Change Order,
<br />in the amount of $8,360. Upon a motion made by Ms. Maradik, seconded by Ms. O'Sullivan and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — CITY OF SOUTH BEND WAYFINDING SIGNAGE
<br />PROGRAM, PHASE 1 — PROJECT NO. 119-052R (WWDA TIF REDA TIF RWDA TIF
<br />PROFESSIONAL SERVICES)
<br />Mr. Gilot advised that Ms. Alicia Czarnecki, Engineering, has submitted Change Order No. 1 on
<br />behalf of Geograph Industries, Inc., 475 Industrial Dr., Harrison, OH 45030, indicating the contract
<br />be extended by sixty-three (63) days for a modified completion date of January 24, 2020. Upon a
<br />motion made by Ms. Maradik, seconded by Ms. O'Sullivan and carried, the Change Order was
<br />approved.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) — MORRIS PERFORMING ARTS CENTER
<br />THEATER HANDRAILS — PROJECT NO. 119-074 (VPA MORRI )
<br />Mr. Gilot advised that Ms. Alicia Czarnecki, Engineering, has submitted Change Order No. 1
<br />(Final) on behalf of Larson Danielson Construction Co., Inc., 302 Tyler St., LaPorte, IN 46350,
<br />indicating the contract amount be increased by $1,250 for a modified contract sum, including this
<br />Change Order, in the amount of $86,810. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — VESTED INTEREST BUILDING ROOF REPAIR —
<br />PROJECT NO. 119-049 (RWDA TIF)
<br />Mr. Gilot advised that Ms. Charlotte Brach, Engineering, has submitted Change Order No. 1 on
<br />behalf of Midland Engineering Company, Inc., 52369 State Road 933 North, South Bend, IN
<br />46637, indicating the contract amount be increased by $21,288.10 for a modified contract sum,
<br />including this Change Order, in the amount of $248,088.10. Upon a motion made by Ms. Maradik,
<br />seconded by Ms. O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 4 — HISTORIC LEEPER PARK IMPROVEMENTS —
<br />PROJECT NO. 118-010A (RWDA TIF BOND)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 4 on behalf
<br />of Ancon Construction Company, Inc., 2146 Elkhart Rd., Goshen, IN 46526, indicating the
<br />contract amount be increased by $12,165.50 for a modified contract sum, including this Change
<br />Order, in the amount of $1,384,979.86. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 3 — WESTERN AVENUE STREETSCAPE, PHASE III —
<br />PROJECT NO. 118-076 (RWDA TIFF
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 3 on behalf
<br />of Indiana Earth, Inc., 10343 McKinley Hwy., Osceola, IN 46561, indicating the contract be
<br />extended by twenty-seven (27) days with a revised completion date of December 27, 2019. Upon
<br />a motion made by Ms. Maradik, seconded by Ms. O'Sullivan and carried, the Change Order was
<br />approved.
