<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 2 (Final) on
<br />behalf of Pemberton -Davis Electric, Inc., 916 E. McKinley Ave., Mishawaka, IN 46545, indicating
<br />the contract amount be decreased by $15,777.37 for a modified contract sum, including this
<br />Change Order, of $172,146.57. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost of $172,146.57. Upon a motion made by Ms. Maradik, seconded by
<br />Ms. O'Sullivan and carried, Change Order No. 2 (Final) and the Project Completion Affidavit
<br />were approved.
<br />APPROVE CHANGE ORDER NO 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of Peerless Midwest, 55860 Russell Industrial Pkwy., Mishawaka, IN 46545, indicating the
<br />contract amount be decreased by $402 for a modified contract sum, including this Change Order,
<br />of $48,598. Additionally submitted was the Project Completion Affidavit indicating this new final
<br />cost of $48,598. Upon a motion made by Ms. Maradik, seconded by Ms. O'Sullivan and carried,
<br />Change Order No. 1 (Final) and the Project Completion Affidavit were approved.

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, IN 46614
<br />indicating the contract amount be decreased by $9,655.45 for a modified contract sum, including
<br />this Change Order, of $384,973.05. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost of $384,973.05. Upon a motion made by Ms. Maradik, seconded by
<br />Ms. O'Sullivan and carried, Change Order No. 1 (Final) and the Project Completion Affidavit
<br />were approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — VPA RESTROOM REMODELING —

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion Affidavit
<br />on behalf of Michiana Contracting, Inc., 7843 Lilac Rd., PO Box 929, Plymouth, IN 46563 for the
<br />above referenced project, indicating a final cost of $529,308.82. Upon a motion made by Ms.
<br />Maradik, seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — PALAIS ROYALE INTERIOR
<br />PAINTING — PROJECT NO. 119-055R (HISTORIC PRESERVATION)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion Affidavit
<br />on behalf of M &.V Pro Services, Inc., 1126 W. Western Ave., South Bend, IN 46601, for the
<br />above referenced project, indicating a final cost of $34,160. Upon a motion made by Ms. Maradik,
<br />seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — HYDRANT FLOW TESTING —
<br />PROJECT NO. 119-041R (WATER WORKS PROFESSIONAL SERVICES)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion Affidavit
<br />on behalf of Shambaugh & Son, LP, 5870 Carbonmill Dr., South Bend, IN 46628, for the above
<br />referenced project, indicating a final cost of $37,145. Upon a motion made by Ms. Maradik,
<br />seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS AND TITLE
<br />SHEET — SEITZ PARK RECONSTRUCTION — PROJECT NO 117-093A (PARK BOND)
<br />In a memorandum to the Board, Ms. Sue Ellen Doudrick, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Also presented at this time for
<br />approval and execution was the Title Sheet. Upon a motion made by Ms. Maradik, seconded by
<br />Ms. O'Sullivan and carried, the above request to advertise was approved, and the Title Sheet was
<br />approved and signed.
