<br />Mr. Gilot advised that Toy Villa, Engineering, has, submitted the Project Completion Affidavit
<br />on behalf of Selge Construction Company, Inc., 2833 S. 1 lth. St., Niles, MI 49120, for the above
<br />referenced project, indicating a final cost of $49,204. Upon a motion made by Ms. Maradik,
<br />seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — THE GARAGE SITE

<br />Mr. Gilot advised that Toy Villa, Engineering, has submitted the Project Completion Affidavit
<br />on behalf of R&R Excavating, Inc., 2010 Went Ave., Mishawaka, IN 46545, for the above
<br />referenced project, indicating a final cost of $59,440. Upon a motion made by Ms. Maradik,
<br />seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was approved.

<br />Mr. Gilot advised that Toy Villa, Engineering, has submitted the Project Completion Affidavit
<br />on behalf of Acorn Landscaping LLC, 3680 W. Sample St., South Bend, IN 46619, for the above
<br />referenced project, indicating a final cost of $136,280.08. Upon a motion made by Ms. Maradik,
<br />seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was approved.

<br />Mr. Gilot advised that Toy Villa, Engineering, has submitted the Project Completion Affidavit

<br />referenced project, indicating a final cost of $76,323.94. Upon a motion made by Ms. Maradik,
<br />seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was approved.

<br />referenced project, indicating a final cost of $99,650. Upon a motion made by Ms. Maradik,
<br />seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT - SCHOOL ZONE FLASHING BEACON
<br />RELOCATIONS — PROJECT NO. 119-058 (LRSA CAPITAL)
<br />Mr. Gilot advised that Toy Villa, Engineering, has submitted the Project Completion Affidavit
<br />on behalf of Michiana Contracting, Inc., PO Box 929, Plymouth, IN 46563, for the above
<br />referenced project, indicating a final cost of $9,800. Upon a motion made by Ms. Maradik,
<br />seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS AND TITLE

<br />In a memorandum to the Board, Roger Nawrot, Engineering, requested permission to advertise
<br />for the receipt of bids for the above referenced project. Also presented at this time for approval
<br />and execution was the Title Sheet. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />O'Sullivan and carried, the above request to advertise was approved, and the Title Sheet was
<br />approved and signed.
