<br />AWARD PROPOSAL — CITY-WIDE PLUMBING CONTRACT (VARIOUS
<br />DEPARTMENTS BUILDING MAINTENANCE ACCOUNTS)
<br />Mr. Adam Parsons, Central Services, advised the Board that on October 22, 2019, proposals
<br />were received and opened for the above referenced contract. After reviewing those proposals,
<br />Mr. Parsons recommended that the Board award the contract to the highest scoring vendor, D.A.
<br />Dodd, 2516 N. Home St., Mishawaka, IN 46545. Therefore, Ms. Maradik made a motion that the
<br />recommendation be accepted, and the proposal be awarded as outlined above. Ms. O'Sullivan
<br />seconded the motion, which carried.
<br />APPROVE CHANGE ORDER NO.2 — 510 S. MAIN STREET BUILDING IMPROVEMENTS
<br />— PROJECT NO. 119-084 (EDIT/GRANTS & SUBSIDIES REDEVELOPMENT GENERAL)
<br />Mr. Gilot advised that Ms. Alicia Czarnecki, Engineering, has submitted Change Order No. 2 on
<br />behalf of HC3 LLC d/b/a St. Clair Development, PO Box 6114, South Bend, IN 46660,
<br />indicating the contract amount be increased by $6,458.50 for a new contract sum, including this
<br />Change Order, in the amount of $161,162,96. Upon a motion made by Ms. Maradik, seconded
<br />by Ms. O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 2 — WESTERN AVE. STREETSCAPE PHASE III —
<br />PROJECT NO. 118-076 (RWDA TIF)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 2 on
<br />behalf of Indiana Earth, Inc., 10343 McKinley Hwy., Osceola, IN 46561, indicating the contract
<br />amount be increased by $133,724.31, plus an additional twenty-two (22) days, for a new contract
<br />sum, including this Change Order, in the amount of $2,809,540.66 and a new project completion
<br />date of November 30, 2019. Upon a motion made by Ms. Maradik, seconded by Ms. O'Sullivan
<br />and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 2 — PULASKI PARK IMPROVEMENTS —PROJECT NO.
<br />118-056 (RWDA TIF BOND)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 2 on
<br />behalf of Larson -Danielson Construction Co., Inc., 302 Tyler St., LaPorte, IN 46350, indicating
<br />the contract amount be increased by $2,284 for a new contract sum, including this Change Order,
<br />in the amount of $880,970.25. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 11 — HOWARD PARK REDEVELOPMENT AND ST.
<br />LOUIS BLVD. IMPROVEMENTS — PROJECT NO 117-047B (PARK
<br />BOND/RETIF/REGIONAL CITIES & CUMULATIVE CAPITAL)
<br />Mr. Gilot advised that Mr. Roger Nawrot, Engineering, has submitted Change Order No. 11 on
<br />behalf of Larson -Danielson Construction Co., Inc., 302 Tyler St., LaPorte, IN 46350, indicating
<br />the contract amount be increased by $157,546.91 for a new contract sum, including this Change
<br />Order, in the amount of $18,604,635.57. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 3 — VPA RESTROOM REMODELING — PROJECT NO.
<br />118-043R (PARKS BOND I)
<br />Mr. Gilot advised that Mr. Scott Kreeger, Engineering, has submitted Change Order No. 3 on
<br />behalf of Michiana Contracting, Inc., 7843 Lilac Rd., PO Box 929, Plymouth, IN 46563,
<br />indicating the contract amount be increased by $4,058, plus an additional twenty-three (23) days,
<br />for a new contract sum, including this Change Order, in the amount of $529,308.82 and a new
<br />project completion date of October 11, 2019. Upon a motion made by Ms. Maradik, seconded by
<br />Ms. O'Sullivan and carried, the Change Order was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — J.C. LAUBER SITE IMPROVEMENTS
<br />—PROJECT NO. 118-083B (REDA TIF)
<br />Mr. Gilot advised that Toy Villa, Engineering, has submitted the Project Completion Affidavit
<br />on behalf of Rieth Riley Construction Co., Inc., 25200 State Road 23, South Bend, IN 46614, for
<br />the above referenced project, indicating a final cost of $416,367.38. Upon a motion made by Ms.
<br />Maradik, seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was
<br />approved.
