Change Order No. 11 to Phase I Contract: CHANGE ORDER NO. 11 <br />This is a request to construct eleven 11 wheelchair TO PHASE I CONTRACT <br />ramps at $300.00 each, for a total increase in contract APPROVED, R -66 <br />amount of $3,300.00, with an increase in contract time <br />of se en (7) calendar days. The unit cost shall include <br />necessary removal of curb and walk, excavation and grad- <br />ing tc conform to detail, placing 3 compacted aggregate <br />base lype "P ", replacing curb and walk as shown in detail <br />for w eelchair ramp, and all necessary labor, equipment <br />and m terials to complete the work. <br />Mr. Taylor advised in discussions with the local chapter <br />of thE National Paraplegic Foundation, we have assured <br />them we would include ramps for handicapped people who <br />must use wheelchairs to enable them to get from one side <br />of thE street to another. This was incorporated into the <br />desigr of the Phase II -B Project, but we need to incorporate <br />it in Phase I. <br />On motion by Mr. Wiggins, seconded by Mr. Wade, Change <br />Order No. 11 to Phase I Contract was approved for an in- <br />creas in contract price of $3,300.00, and an increase <br />of se en (7) calendar days for contract time completion. <br />1. Change Order Nc. 1 to Demolition Contract No. 6: CHANGE ORDER NO. 1 <br />This hange Order is to add a house which had burned, TO DEMOLITION CONTRACT <br />locat d at 138 South Meade Street, Parcel 1 -18, to the NO. 6 APPROVED, R -57 <br />demolition contract. The demolition cost for this <br />house is $500.00, bringing the total demolition contract <br />to $3 686.00, subject to Legal Counsel approval and HUC <br />concu rence. <br />Mr. Taylor advised we have been under extreme pressure <br />by the PAC Committee, in LaSalle Park, to remove this <br />structure, but we had to get concurrence from HUD to <br />acquire the property, which was included in the amenda- <br />tory, but we have not received formal notice of approval. <br />On motion by Mr. Wade, seconded by Mr. Wiggins, Change <br />Order No. 1 to Demolition Contract No. 6, was approved <br />in th amount of $500.00, which is an increase to the <br />contract price, subject to Legal Counsel approval and <br />HUD concurrence.
