REGULAR MEETING SEPTEMBER 24, 2019 286 <br />APPROVE PROJECT COMPLETION AFFIDAVIT — CENTURY CENTER CONVENTION <br />HALL A-B NEW ENTRY — PROJECT NO 115-017A (HOTEL/MOTEL TAX) <br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion <br />Affidavit on behalf of Brown & Brown General Contractors, Inc., 124 S. Elkhart St., Wakarusa, <br />IN 46573, for the above referenced project, indicating a final cost of $96,325. Upon a motion <br />made by Ms. Maradik, seconded by Ms. Dorau and carried, the Project Completion Affidavit <br />was approved. <br />APPROVE PROJECT COMPLETION AFFIDAVIT — CENTURY CENTER UPGRADES <br />AND RENOVATIONS — PROJECT NO 118-104AR (HOTEL/MOTEL TAX) <br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion <br />Affidavit on behalf of R. Yoder Construction, Inc., 27453 CR 150, Nappanee, IN 46550, for the <br />above referenced project, indicating a final cost of $104,180. Upon a motion made by Ms. <br />Maradik, seconded by Ms. Dorau and carried, the Project Completion Affidavit was approved. <br />APPROVE PROJECT COMPLETION AFFIDAVIT — ERSKINE WELL VARIABLE <br />FREQUENCY DRIVE INSTALLATION — PROJECT NO 119-032 (SSDA TIF) <br />Mr. Gilot advised that Mr. Zach Hurst, Engineering, has submitted the Project Completion <br />Affidavit on behalf of Herrman & Goetz, Inc., 225 S. Lafayette Blvd., South Bend, IN 46601, for <br />the above referenced project, indicating a final cost of $6,175. Upon a motion made by Ms. <br />Maradik, seconded by Ms. Dorau and carried, the Project Completion Affidavit was approved. <br />APPROVE PROJECT COMPLETION AFFIDAVIT — WASTEWATER TREATMENT <br />PLANT HEADWORKS INFLUENT GATES IMPROVEMENTS — PROJECT NO. 117-026 <br />(WASTEWATER CAPITAL ACCOUNT) <br />Mr. Gilot advised that Mr. Jacob Klosinski, Utilities, has submitted the Project Completion <br />Affidavit on behalf of HRP Construction, Inc., 5777 Cleveland Rd., South Bend, IN 46628, for <br />the above referenced project, indicating a final cost of $635,731.06. Upon a motion made by Ms. <br />Maradik, seconded by Ms. Dorau and carried, the Project Completion Affidavit was approved. <br />BOARD MEMBER EXCUSED <br />Ms. O'Sullivan stepped out of the meeting at 10:02 a.m. <br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS 2019 <br />DEMOLITIONS, PHASE 2 — PROJECT NO 119-079 (VACANT AND ABANDONED) <br />In a memorandum to the Board, Ms. Charlotte Brach, Engineering, requested permission to <br />advertise for the receipt of bids for the above referenced project. Therefore, upon a motion made <br />by Ms. Maradik, seconded by Ms. Dorau and carried, the above request was approved. <br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS CITY <br />UNIFORMS. MATS AND RUGS (MULTIPLE CITY DEPARTMENTS <br />In a memorandum to the Board, Mr. Michael Schmidt, Purchasing, requested permission to <br />advertise for the receipt of bids for the above referenced materials. Therefore, upon a motion <br />made by Ms. Maradik, seconded by Ms. Dorau and carried, the above request was approved. <br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS AND TITLE <br />SHEET — HISTORIC LEEPER PARK IMPROVEMENTS — PROJECT NO 118 O1 OB (RWDA <br />TF <br />In a memorandum to the Board, Mr. Kyle Silveus, Engineering, requested permission to <br />advertise for the receipt of bids for the above referenced project. Also presented at this time for <br />approval was the Title Sheet.
