<br />Ms. Dorau and carried, Change Order No, 2 (Final) and the Project Completion Affidavit were

<br />APPROVE CHANGE ORDER NO 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— MAIN AND JEFFERSON EARTHWORK — PROJECT NO 119-022 (RWDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 (Final) on
<br />behalf of Hardman Construction, Inc., 242 South Brye Road, Ludington, MI 49431, indicating
<br />the contract amount be decreased by $9,750 for a new contract sum, including this Change
<br />Order, of $255,250. Additionally submitted was the Project Completion Affidavit indicating this
<br />new final cost of $255,250. Upon a motion made by Ms. Maradik, seconded by Ms. Dorau and
<br />carried, Change Order No. 1 (Final) and the Project Completion Affidavit were approved.
<br />APPROVE CHANGE ORDER NO 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— FARMINGTON 14A LIFT STATION REHABILITATION — PROJECT NO. 117-027
<br />(UTILITIES, WASTEWATER)
<br />Mr. Gilot advised that Mr. Jacob Klosinski, Utilities, has submitted Change Order No. 1 (Final)
<br />on behalf of John Boettcher Sewer & Excavating, Inc.,3305 North Home St., Mishawaka, IN
<br />46545 indicating the contract amount be decreased by $23,125.23, plus an additional one -
<br />hundred, forty (140) days, for a new contract sum, including this Change Order, of $337,452.56
<br />and a new project completion date of September 30, 2018. Additionally submitted was the
<br />Project Completion Affidavit indicating this new final cost of $337,452.56. Upon a motion made
<br />by Ms. Maradik, seconded by Ms. Dorau and carried, Change Order No. 1 (Final) and the Project
<br />Completion Affidavit were approved.

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Bokon Masonry, Inc., 56571 Pear Road, South Bend, IN 46619, for the
<br />above referenced project, indicating a final cost of $237,713. Upon a motion made by Ms.
<br />Maradik, seconded by Ms. Dorau and carried, the Project Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — UNITY GARDENS LEARNING
<br />CENTER SITE IMPROVEMENTS — PROJECT NO 117-05OR (RWDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Ritschard Bros., Inc., 1204 W. Sample St., South Bend, IN 46619, for the
<br />above referenced project, indicating a final cost of $73,541.85. Upon a motion made by Ms.
<br />Maradik, seconded by Ms. Dorau and carried, the Project Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — THREE TWENTY AT THE CASCADE
<br />COMPOSITE WALL PANELS DIVISION A — PROJECT NO 118-074C (REDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Construction Ahead, Inc., 1659 N. Lancaster Rd., South Elgin, IL 60177,
<br />for the above referenced project, indicating a final cost of $174,729. Upon a motion made by Ms.
<br />Maradik, seconded by Ms. Dorau and carried, the Project Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — CENTURY CENTER SITE
<br />IMPROVEMENTS — PROJECT NO 118-101 (HOTEL/MOTEL TAX)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Acorn Landscaping LLC, 3680 W, Sample St., South Bend, IN, for the
<br />above referenced project, indicating a final cost of $136,280.08. Upon a motion made by Ms.
<br />Maradik, seconded by Ms. Dorau and carried, the Project Completion Affidavit was approved.

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Dudeck Roofing and Sheet Metal, 1634 Franklin St., South Bend, IN
<br />46613, for the above referenced project, indicating a final cost of $133,295. Upon a motion made
<br />by Ms. Maradik, seconded by Ms. Dorau and carried, the Project Completion Affidavit was
<br />approved.
