<br />Mr. Gilot advised that Mr. Scott Kreeger,: Engineering, has submitted Change Order No. 2 on
<br />behalf of Michiana Contracting, Inc., 7843 Lilac Road, PO Box 929, Plymouth, IN 46563,
<br />indicating the contract completion date be increased by nineteen (19) days for a new contract
<br />completion date of September 18, 2019. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — WESTERN AVENUE STREETSCAPE PHASE III —
<br />PROJECT NO. 118-076 (RWDA TIF/STORM WATER FUND)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 1 on
<br />behalf of Indiana Earth, Inc., 10343 McKinley Hwy., Osceola, IN 46561, indicating the contract
<br />amount be increased by $41,431 and a time extension of four (4) days, for a new contract sum,
<br />including this Change Order, in the amount of $2,675,816.35 and a new completion date of
<br />November 8, 2019. Upon a motion made by Ms. Maradik, seconded by Ms. O'Sullivan and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 2— NORTH STATION WELL NO. 1 REPLACEMENT —
<br />PROJECT NO. 117-031R (WATER WORKS)
<br />Mr. Gilot advised that Ms. Becca Plantz, Engineering, has submitted Change Order No. 2 on
<br />behalf of Peerless Midwest, Inc., 55860 Russell Industrial Parkway, Mishawaka, IN 46545,
<br />indicating the contract amount be increased by $1,345 for a new contract sum, including this
<br />Change Order, in the amount of $543,570.72. Upon a motion made by Ms. Maradik, seconded
<br />by Ms. O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 2 — LIBERTY TOWER IMPROVEMENTS, DIVISION C
<br />—PROJECT NO. 118-086R (RWDA TIF)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 2 on
<br />behalf of Environmental Glass, Inc., 12675 Marlin Dr., Redford, MI 48239, indicating the
<br />contract amount be increased by $2,219.78, plus an additional fourteen (14) days for a new
<br />contract sum, including this Change Order, in the amount of $301,992.46 and a new project
<br />completion date of September 13, 2019. Upon a motion made by Ms. Maradik, seconded by
<br />Ms. O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 3 — LIBERTY TOWER IMPROVEMENTS, DIVISION C
<br />— PROJECT NO. 118-086R (RWDA TIF)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 3 on
<br />behalf of Environmental Glass, Inc., 12675 Marlin Dr., Redford, MI 48239, indicating the
<br />contract amount be increased by $3,813.31 for a new contract sum, including this Change Order,
<br />in the amount of $305,805.77. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 2 — LAFAYETTE BUILDING EXTERIOR
<br />RENOVATION, DIVISION A — PROJECT NO. 117-100R (RWDA TIF)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 2 on
<br />behalf of Precision Wall Systems, Inc., 3801 S. Main Street, South Bend, IN 46614, indicating
<br />the contract amount be increased by $20,229, plus an additional sixty-six (66) days, for a new
<br />contract sum, including this Change Order, in the amount of $237,812 and a new project
<br />completion date of September 30, 2019. Upon a motion made by Ms. Maradik, seconded by
<br />Ms. O'Sullivan and carried, the Change Order was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — CODE ENFORCEMENT DOOR
<br />INSTALLATION (CODE ENFORCEMENT)
<br />Mr. Gilot advised that Mr. Adam Parsons, Central Services, has submitted the Project
<br />Completion Affidavit on behalf of Brown & Brown General Contractors, Inc., 124 S. Elkhart
<br />Street, Wakarusa, IN 46573, for the above referenced project, indicating a final cost of $3,148.
<br />Upon a motion made by Ms. Maradik, seconded by Ms. O'Sullivan and carried, the Project
<br />Completion Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — LINCOLN WAY WEST AND
<br />CHARLES MARTIN SR. DR. IMPROVEMENTS — PROJECT NO 117-072A (RWDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Selge Construction Co., Inc., 2833 S. 111h St., Niles, MI 49120, for the
<br />above referenced project, indicating a final cost of $1,197,648.10. Upon a motion made by Ms.
