BOWEN SAFETY POLICY MANUAL <br />F. Employee Assistance Program — For Employees Classified as part of Bowen <br />Management Staff <br />1. We recognize that it is sometimes difficult to cope successfully with the <br />pressures that life puts on us, and that personal problems can adversely <br />affect your ability to perform satisfactorily on the job. <br />2, We have therefore, implemented an Employee Assistance Plan to provide <br />counseling and referral services to employees who are having such <br />problems and voluntarily choose to use this program. The employee or <br />dependent does not have to notify Bowen for approval to contact this <br />referral service. If treatment necessitates time away from work, Bowen <br />may provide for the employee an unpaid leave of absence, not to exceed <br />30 days, for purposes of participation in an agreed upon treatment <br />program. During this 30-day period an employee who seeks treatment, <br />and successfully completes a rehabilitation program, or is in the process <br />of completing such a program, may be reinstated to their former <br />employment status, if work for which they are qualified is available at that <br />time. For employees that choose to take a leave of absence prior to <br />returning to work, must submit written documentation from a Substance <br />Abuse Professional (SAP) of completion or the present participation in, a <br />rehabilitation program, and submit a negative drug test. <br />3. Confidentiality is of the highest concern at Bowen for those seeking help <br />for their personal problems. Bowen encourages those in need to <br />participate in this program. You may reach this service, at no cost to you, <br />by placing a call to One to One Employee Assistance at 1-800-745-4838, in <br />order to set an appointment. The cost of five (5) sessions is at no charge. <br />4. You should understand, however, that our firm has no part in the <br />operation of this program. It is an independent consulting agency, and we <br />therefore assume no responsibility or liability for any of its acts or <br />omissions. In the event of a non -negative drug or alcohol test result an <br />initial mandatory referral to Bowen's EAP will occur. <br />5. The cost of a rehabilitation program may be the responsibility of the <br />employee. <br />G. Conclusion <br />L This program and policy statement is intended to protect our most <br />valuable asset, our employees. The health and safety of all employees and <br />the general public is of utmost concern. The above presented policy will <br />help insure a safe DRUG -FREE WORKPLACE for all. <br />2.
