Municipal Utilities and Service Line _----------- Rules and Rg glens <br />full amount due (financial hardship shall constitute <br />cause), and said customer: <br />(1) Pays a reasonable portion of at least ten dollars <br />($10.00); and <br />(2) Agrees to pay the remainder of the outstanding bill <br />within three (3) months; and <br />(3) Has not breached any similar agreement with the <br />Utility made pursuant to this rule within the past <br />twelve (12) months. <br />d) The Utility may in its sole judgment grant an extension of <br />time for the payment of the shut-off amount. The Utility <br />may grant or deny an extension agreement based on past <br />payment history or any other information deemed <br />pertinent. The following conditions must be met in order <br />to receive a payment extension: <br />(1) All requests for extensions must be made by the <br />shut-off date. <br />(2) No extension agreements may be made with field <br />operations personnel. <br />(3) No extensions will be granted while a current <br />extension agreement is in force at the subject <br />address. <br />(4) Failure to meet the terms of an extension <br />agreement will cause such document to become <br />void and make all past due bills to become <br />immediately due and subject to disconnection at <br />the discretion of the Utility. <br />(5) Customers are limited to four (4) extensions per <br />calendar year. If the extension agreement is not <br />honored, the customer is not eligible for another <br />extension for one (1) year from date of violation. <br />(6) Extensions may be granted for up to ten (10) <br />calendar days from the shut-off date. <br />2. The Utility shall postpone the disconnection of service for ten <br />(10) days if, prior to the disconnection date specified in the <br />disconnect notice; <br />a) The customer provides the Utility with a medical <br />statement from a licensed physician or public health <br />official; and <br />b) Notification must state, that disconnection of service <br />would be a serious and immediate threat to the health or <br />safety of a designated person in the household of the <br />26 <br />
