High Concrete Group LLC Date: May 28, 2019 <br />Job Name: Commerce Center Estimate: 19-03-033 <br />ACCESS & INSTALLATION SPECIFIC QUALIFICATIONS: <br />1. EQUIPMENT ACCESS: HCG delivery trucks must have clear and unobstructed access within reach of the erection <br />crane(s) and trucks AS SHOWN IN. THE ATTACHED SITE LOGISITCS PLAN PREPARED BY HIGH CONCRETE <br />BASED ON SITE VISIT. Access for precast delivery trucks must be firm and level so equipment can move under its <br />own power to the crane location. 30-foot access must be supplied and maintained by others, not HCG. Overhead <br />restrictions on the site; or at the site access entry point must be removed prior by others at no cost to HCG. <br />2. All coordination, logistic, permits, construction barriers, power/telephone line relocation, etc. required for the access <br />into the site will be the responsibility of others. Site logistics are to be provided by others. <br />3. HCG will seek an area on site for staging 3-4 trailers during erection to facilitate dropped loads; an area immediately <br />adjacent to the building footprint will also be utilized for movement of deliveries. <br />4. Any site work required creating acceptable grading for trucks must be by the GC. <br />5. CRANE MOBILIZATION(s) INCLUDED: 1 (ONE) for ground crane/crew to be erected continuous. <br />6. SITE LABOR: Erector — OPEN SHOP LABOR Patching Crew — Open Shop LABOR. All installation is to be <br />performed during daylight normal working hours Monday through Friday. No overtime or weekend work is included. <br />7. TEMPORARY UTILITIES: Purchaser shall provide HCG with temporary 100v and 120v electric, and water under <br />pressure within 50 feet of our work. <br />8. If second shift work is required, sufficient lighting must be provided by others to permit adequate visual control of all <br />installation operations. Please note that second shift work is not provided as part of this proposal and shall be <br />negotiated with the HCG Project Manager. <br />9. SAFETY CABLES AND/OR GUARDRAIL SYSTEMS: (consisting of both top and mid -height rails as well as toe <br />boards, which abide by O.S.H.A. Standards), must be furnished, installed and maintained along the perimeter of all <br />roof and floor areas, floor openings, etc. by others. Any perimeter safety devices, (i.e. cables, guardrails, stanchions, <br />toe boards, etc), which interfere with the actual location in which precast panels are to be erected, shall be removed <br />by HCG personnel as needed. <br />10. ERECTION SAFETY: The erection operation will utilize an OSHA approved Monitored Safety Plan developed by <br />HCG's erection subcontractor. HCG erectors to abide by 100% tie -off above 6 feet. Tie off is not included on any <br />trailer deck unloading operation. <br />11. LOADING, DELIVERING AND SHUTTLING: of all precast concrete units are to be in accordance with federal, state <br />and local laws, and PCI Standards, Division 2, Practices and Procedures. <br />CONTRACT SPECIFIC QUALIFICATIONS <br />HCG qualifies that modifications to the Contract Agreement and General Conditions must be incorporated based on mutually <br />agreeable terms between HCG and the Contractor. The changes typically relate to issues including but not limited to: <br />indemnification, insurance, payment terms for stored material, retainage, and Subcontractor obligations. Other items are also <br />included in this section. HCG will also require specific language to address Owner's Termination for Convenience; specific <br />language must be added to the Subcontract Rider addressing HCG's lost opportunity and profits in the event of termination <br />for convenience by the Owner. <br />PAYMENT FOR STORED MATERIALS <br />Due to the nature and size of this project, a majority of the precast product will need to be made months in advance to ensure <br />enough product is available when the site requires it. Because of this necessity, HCG asks that payment for finished and <br />yard -stored precast panels be made due to the excessive costs that would be "fronted" for this project. Photographic and <br />written documentation of each panel completed and stored can be provided to assist with the payment process if needed. <br />Should this payment process be denied, HCG would have additional extensive costs that are not covered within this proposal <br />and need to be revisited. <br />4 <br />
