<br />APPROVE CHANGE ORDER NO. 4 — LINCOLNWAY WEST AND CHARLES MARTIN
<br />SR. DRIVE IMPROVEMENTS — PROJECT NO. 117-072A (RWDA TIF)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 4 on
<br />behalf of Selge Construction Co., Inc., 2833 S IIth Street, Niles, MI 49120, indicating the
<br />contract amount be increased by $3,260.07 for a new contract sum, including this Change Order,
<br />in the amount of $1,198,523.10. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 2 — CENTURY CENTER CONVENTION HALL A-B
<br />ENTRY —PROJECT NO. 115-017A (HOTEL/MOTEL TAX)
<br />Mr. Gilot advised that Mr. Roger Nawrot, Engineering, has submitted Change Order No. 2 on
<br />behalf of Brown & Brown General Contractors, Inc., 124 S. Elkhart St., Wakarusa, IN 46573,
<br />indicating the contract amount be increased by $7,911 for a new contract sum, including this
<br />Change Order, in the amount of $96,325. Upon a motion made by Ms. Maradik, seconded by
<br />Ms. O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — CENTURY CENTER STORAGE MEZZANINE —
<br />PROJECT NO. 118-036E (HOTEL/MOTEL TAX)
<br />Mr. Gilot advised that Ms. Becca Plantz, Engineering, has submitted Change Order No. 1 on
<br />behalf of R. Yoder Construction, Inc., 27453 County Road 150, Nappanee, IN 46550, indicating
<br />the contract amount be increased by $243.52 for a new contract sum, including this Change
<br />Order, in the amount of $104,243.52. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 5 — FIRE STATION NO. 9 — PROJECT NO. 116-012

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 5 on behalf
<br />of Ziolkowski Construction, Inc., 4050 Ralph Jones Dr., South Bend, IN 46628, indicating the
<br />contract amount be decreased by $553, plus an additional fifteen (15) days, for a new contract
<br />sum, including this Change Order, in the amount of $3,644,549 and a new project completion
<br />date of June 14, 2019. Upon a motion made by Ms. Maradik, seconded by Ms. O'Sullivan and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — SOUTH BEND FIRE DEPARTMENT TRAINING
<br />CENTER CLASSROOM BUILDING — PROJECT NO. 117-127R (2018 FIRE STATION NO. 9

<br />Mr. Gilot advised that Ms. Alicia Czarnecki, Engineering, has submitted Change Order No. 1 on
<br />behalf of Brown & Brown General Contractors, Inc., 124 S. Elkhart St., Wakarusa, IN 46573,
<br />indicating the contract amount be increased by $2,151, plus an additional twenty (20) days for a
<br />new contract sum, including this Change Order, in the amount of $652,088 and a new project
<br />completion date of October 20, 2019. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />O'Sullivan and carried, the Change Order was approved.

<br />Mr. Gilot advised that Mr. Adam Parsons, Sustainability, has submitted Change Order No. 1 on
<br />behalf of Brown & Brown General Contractors, Inc., 124 S. Elkhart St., Wakarusa, IN 46573,
<br />indicating the contract amount be increased by $4,118 for a new contract sum, including this
<br />Change Order, in the amount of $8,894. Upon a motion made by Ms. Maradik, seconded by
<br />Ms. O'Sullivan and carried, the Change Order was approved.

<br />Mr. Gilot advised that Mr. Adam Parsons, Sustainability, has submitted Change Order No. 1 on
<br />behalf of Premier 1 Construction, Inc., 105 E. Jefferson Blvd., South Bend, IN 46601, indicating
<br />the contract amount be increased by $2,877 for a new contract sum, including this Change Order,
<br />in the amount of $24,129. Upon a motion made by Ms. Maradik, seconded by Ms. O'Sullivan
<br />and carried, the Change Order was approved.
