<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 12 on
<br />behalf of Ziolkowski Construction, Inc., 4050 Ralph Jones Dr., South Bend, IN 46628, indicating
<br />the contract amount be increased by $9,811 for a new contract sum, including this Change Order,
<br />in the amount of $1,101,342. Upon a motion made by Ms. Maradik, seconded by Ms. O'Sullivan
<br />and. carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 - CLEVELAND WELL FIELD REFURBISHMENTS -
<br />PROGRAM RELATED ITEMS — PROJECT NO. 118-021A (RWDA TIF)
<br />Mr. Gilot advised that Ms. Becca Plantz, Engineering, has submitted Change Order No. 1 on
<br />behalf of Peerless Midwest, Inc., 55860 Russell Industrial Parkway, Mishawaka, IN 46545,
<br />indicating the contract amount be increased by $3,894 for a new contract sum, including this
<br />Change Order, in the amount of $140,744. Upon a motion made by Ms. Maradik, seconded by
<br />Ms.. O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 — JMS BUILDING PARKING LOT IMPROVEMENTS
<br />— PROJECT NO. 118-003R (RWDA TIF)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 1 on
<br />behalf of Milestone Fence, LLC, 13399 McKinley Highway, Mishawaka, IN 46545, indicating
<br />the contract amount be increased by $1,,150, plus an additional one -hundred seventy-three (173)
<br />days, for a new contract sum, including this Change Order, in the amount of $85,831 and a new
<br />project completion date of May 1, 2019. Upon a motion made by Ms. Maradik, seconded by
<br />Ms. O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 2 (FINAL) AND PROJECT COMPLETION AFFIDAVIT

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 2 (Final) on
<br />behalf of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, IN 46614,
<br />indicating the contract amount be decreased by $5,429.54 for a new contract sum, including this
<br />Change Order, of $547,774.39. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost of $547,774.39, Upon a motion made by Ms. Maradik, seconded by
<br />Ms. O'Sullivan and carried, Change Order No. 2 (Final) and the Project Completion Affidavit
<br />were approved.

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Gibson -Lewis, LLC, 1001 W. 11th Street, Mishawaka, IN 46544, for the
<br />above referenced project, indicating a final cost of $3,351,062. Upon a motion made by Ms.
<br />Maradik, seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was
<br />approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — LIGHT UP SOUTH BEND -RIVERSIDE
<br />LIGHTING — PROJECT NO. 118-011A (COIT)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Martell Electric, LLC, 4601 Cleveland Road, South Bend, IN 46628, for
<br />the above referenced project, indicating a final cost of $103,189.20. Upon a motion made by Ms.
<br />Maradik, seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was
<br />approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — ZIKER SAMPLE STREET BUILDING
<br />RESTROOM REMODEL — PROJECT NO. 117-123A (RWDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Premier 1 Construction, 105 E. Jefferson Blvd., Suite 216, South Bend, IN
<br />46601, for the above referenced project, indicating a final cost of $45,976. Upon a motion made
<br />by Ms. Maradik, seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was
<br />approved.
