<br />upon a motion made by Ms. Maradik, seconded by Ms. O'Sullivan and carried, the above request
<br />was approved.
<br />AWARD QUOTATION — 2019 VENUES PARKS AND ARTS CEMETERY MOWING
<br />(VPA MAINTENANCE)
<br />Mr. Patrick Sherman, Venues, Parks and Arts, advised the Board that on March 12, 2019,
<br />quotations were received and opened for the above referenced project. After reviewing those
<br />quotations, Mr. Sherman recommended that the Board award the contract to the lowest
<br />responsive and responsible bidder, Michiana's Four Seasons Landscaping, 24449 Rolling Oaks
<br />Drive, South Bend, IN 46628, in the amount of $43,180 for the Base Quote plus Alternate No. 1.
<br />Therefore, Ms. Maradik made a motion that the recommendation be accepted and the quotation
<br />be awarded as outlined above. Ms. O'Sullivan seconded the motion, which carried.
<br />AWARD QUOTATION — LOT 12 US 31 INDUSTRIAL - LANDSCAPE WORK — PROJECT
<br />NO. 119-029 (RWDA TIF)
<br />Mr. Zach Hurst, Engineering, advised the Board that on March 12, 2019, quotations were
<br />received and opened for the above referenced project. After reviewing those quotations, Mr.
<br />Hurst recommended that the Board award the contract to the lowest responsive and responsible
<br />bidder, Fuerbringer Landscaping and Design, Inc., 22530 Brick Rd., South Bend, IN 46628, in
<br />the amount of $52,110. Therefore, Ms. Maradik made a motion that the recommendation be
<br />accepted and the quotation be awarded as outlined above. Ms. O'Sullivan seconded the motion,
<br />which carried.
<br />AWARD QUOTATION — CENTURY CENTER SITE IMPROVEMENTS — PROJECT NO.
<br />118-101 (HOTEL/MOTEL TAX)
<br />Ms. Becca Plantz, Engineering, advised the Board that on March 12, 2019, quotations were
<br />received and opened for the above referenced project. After reviewing those quotations, Ms.
<br />Plantz recommended that the Board award the contract to the lowest responsive and responsible
<br />bidder, Acorn Landscaping, LLC, 3680 W. Sample Street, South Bend, IN 46619, in the amount
<br />of $131,069.02 for the base quote only. Therefore, Ms. Maradik made a motion that the
<br />recommendation be accepted and the quotation be awarded as outlined above. Ms. O'Sullivan
<br />seconded the motion, which carried.
<br />APPROVE CHANGE ORDER NO. 1 — MIAMI STREET BASIN DRAINAGE
<br />IMPROVEMENTS —PROJECT NO 116-082C (SSDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1 on behalf
<br />of HRP Construction, Inc., 5777 Cleveland Road, South Bend, IN 46624, indicating the contract
<br />amount be increased by $8,780, plus an additional one -hundred, thirty-nine (139) days for a new
<br />contract sum, including this Change Order, in the amount of $501,780 and a new project
<br />completion date of May 12, 2019. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />O'Sullivan and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 10 — ERSKINE CLUBHOUSE RENOVATION —
<br />PROJECT NO. 116-108 (VPA CAPITAL)
<br />Mr. Gilot advised that Mr. Kyle Silveus, Engineering, has submitted Change Order No. 10 on
<br />behalf of Ziolkowski Construction, Inc., 4050 Ralph Jones Drive, South Bend, IN 46628,
<br />indicating the contract amount be increased by $14,689 for a new contract sum, including this
<br />Change Order, in the amount of $1,091,531. Upon a motion made by Ms. Maradik, seconded by
<br />Ms. O'Sullivan and carried, the Change Order was approved. Mr. Gilot noted the Board was
<br />informed that the majority of the changes on this project are due to unforeseen differing site
<br />conditions, discovered during excavation.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — EDISON PRODUCTION WELL NO.
<br />2A REHABILITATION — PROJECT NO 118-103 (WATER WORKS O&M)
<br />Mr. Gilot advised that Ms. Becca Plantz, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Peerless -Midwest, Inc., 55860 N. Russell Industrial Parkway, Mishawaka,
<br />IN 46545, for the above referenced project, indicating a final cost of $63,140. Upon a motion
<br />made by Ms. Maradik, seconded by Ms. O'Sullivan and carried, the Project Completion
<br />Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — OLIVE PRODUCTION WELL NO. 6
<br />REHABILITATION — PROJECT NO 118-102 (WATER WORKS 0&M)
<br />Mr. Gilot advised that Ms. Becca Plantz, Engineering, has submitted the Project Completion
