REGULAR MEETING JULY 18, 2012 55 <br />Declaration for Retirement for Arson Investigator Rudy Jimenez effective June 21, 2012. Chief Cox <br />noted that Mr. Jiminez has served the Department with loyalty and diligence during the last twenty - <br />six (26) years, and nine (9) months. Chief Cox joined the Board in thanking him for his commitment, <br />dedication and years of distinguished service to the South Bend Fire Department and the citizens of <br />South Bend. Upon a motion made by Mr. Miller, seconded by Mr. Taylor and carried, the retirement <br />was accepted and approved. <br />ACCEPT AND APPROVE RETIREMENT — TIMOTHY JANOWIAK <br />President Cottrell advised that Fire Chief Steve Cox has submitted to the Board the Application and <br />Declaration for Retirement for Pump Engineer Timothy Janowiak effective June 30, 2012. Chief <br />Cox noted that Mr. Janowiak has served the Department with loyalty and diligence during the last <br />twenty -three (23) years, and nine (9) months. Board Member and former Fire Chief Luther Taylor <br />stated he would like to acknowledge Tim for his great ability to work with the public on special <br />projects. He noted Mr. Janowiak hit the ground running when he came on the department. Mr. Taylor <br />added that Mr. Janowiak was instrumental in getting the Fire Fighter's Memorial built, purchasing <br />the Fire Safety House, and creation of the Fire Fighter's Blues Fest. He stated Tim was a great asset <br />to the department. Chief Cox joined the Board in thanking him for his commitment, dedication and <br />years of distinguished service to the South Bend Fire Department and the citizens of South Bend. <br />Upon a motion made by Mr. Miller, seconded by Mr. Taylor and carried, the retirement was accepted <br />and approved. <br />ACCEPT AND APPROVE RETIREMENT — ALFRED KIRSITS <br />President Cottrell advised that Fire Chief Steve Cox has submitted to the Board the Application and <br />Declaration for Retirement for Battalion Chief Alfred Kirsits effective June 27, 2012. Chief Cox <br />noted that Mr. Kirsits has served the Department with loyalty and diligence during the last thirty -two <br />(32) years, and six (6) months. Chief Cox joined the Board in thanking him for his commitment, <br />dedication and years of distinguished service to the South Bend Fire Department and the citizens of <br />South Bend. Upon a motion made by Mr. Miller, seconded by Mr. Taylor and carried, the retirement <br />was accepted and approved. <br />ACCEPT AND APPROVE RETIREMENT — GREGORY MACK <br />President Cottrell advised that Fire Chief Steve Cox has submitted to the Board the Application and <br />Declaration for Retirement for Lieutenant Gregory Mack effective July 9, 2012. Chief Cox noted <br />that Mr. Mack has served the Department with loyalty and diligence during the last twenty -one (2 1) <br />years, and three (3) months. Chief Cox joined the Board in thanking him for his commitment, <br />dedication and years of distinguished service to the South Bend Fire Department and the citizens of <br />South Bend. Upon a motion made by Mr. Miller, seconded by Mr. Collins and carried, the retirement <br />was accepted and approved. <br />APPROVE AND ENTER FINDINGS OF FACT — _CHARGE NO. 2012-03 — THOMAS SHEETS <br />Attorney Andrea Beachkofsky presented the Findings of Facts for Charges No. 2012 -03 for the <br />Board's approval, stating that Captain Thomas Sheets be suspended without pay from the South <br />Bend Fire Department for a period of thirty (30), eight (8) hour days to begin on a date chosen by the <br />Chief and that he be removed permanently from the position of self - contained breathing apparatus <br />technician for the Department indefinitely. Upon a motion by Mr. Miller, seconded by Mr. Taylor <br />and carried, the Findings of Fact were approved and entered as outlined above. <br />FILING OF QUARTERLY TRAVEL AND TRAINING EXPENDITURES REPORT — APRIL <br />MAY, AND JUNE 2012 <br />Fire Chief Steve Cox submitted to the Board a written Quarterly Travel and Training Expenditures <br />Report for the period of April, May, and June 2012. The report summarizes travel and training <br />expenditures for the South Bend Fire Department personnel during this quarterly period. In his letter, <br />Chief Cox stated that training is essential to all aspects of fire service and it is necessary to remain <br />abreast of the most current firefighting technologies and emergency medical services. There being no <br />questions concerning this report, President Cottrell stated the Quarterly Report for Travel and <br />Training Expenditures would be accepted for filing. <br />FILING OF MONTHLY STATISTICAL ANALYSIS REPORT — JUNE 2012 <br />Fire Chief Stephen Cox submitted to the Board the monthly report for the month of June, 2012 <br />compiled by the Prevention/Inspection Bureau of the Fire Department.
